Question: Hi. I had very painfull Period , so my doctor advised me to take Duphaston from 5-25 day of my cycle. She also said after last pill I should start mensruating on 28-30 day of my cycle. On 28 I got very light spot, but no menses. Now Is 49 day of my cycle and still no menses>>>I took pregnancy test on 32 day of my cycle , it was negative... Is there any posibilities that my ovulation moved during taking Duphaston? Thanks

Answer: Duphaston may cause a scanty period. So, the bleeding you had maybe the period. have you done an ultrasound to rule out a fibroid or an ovarian cyst which maybe causing you the pain?

Question: I have just delieverd 10days back - csection, just wanted ur advise if disflatyl tabs are good for breast feeding mothers? also wanted to know how could i increase the milk(breast)currently not much, appetite is not good, thanks.

Answer: Disflatyl is safe during breast feeding. If you are in Oman, you can take an appointment with me- 24563181 , and I can examine you and advice you further regarding the breast feeding.

Question: My Name is MArium, and im 25 years old. I got married 1 year and a half ago, and havent conceived yet as according to my reports, my progesterone level is low. My question is, the liquid that our body discharges around our ovulation, i think its called cervix mucus if im not wrong, then am i supposed to have enough discharge to notice on my panties to determine if i am ovulating properly?? Also, i have taken prolifen this month and i did not discharge as i think i should have around ovulation. What are the other signs of ovulation? Please help me with some tips on conceiving. The doctors say im fine.

Answer: Progesterone level should be done around the 20th - 22nd day of the period. If it was low then , it indicates that you did not ovulate during that cycle. Regarding cervical mucus- the cervix produces mucus throughout the cycle. However, this is increased during ovulation. The amount varies from person to person and that alone would not indicate when you have ovulated. Ultrasound would be motre reliable, as it would show the size of the follicle, the number of follicles and therefore, the most fertile period. Prolifen may reduce the amount of cervical mucus. is your husband's semen mnormal nad are your tubes patent?

Question: I had an iui done yesterday at 10:00am. (i was on folistim injectables) then last night around 9:00pm I noticed a small blob of brown blood (very little) mixed with thick transparent cervical mucose as I wiped my cm is back to normal now. Could the cause be ovulating follicles (the egg realeasing) Thank you so much.

Answer: I cannot answer your questions the same day, as your questions go the omanaccess website and then are sent ot me collectively. I then send the naswerrs to the website, si it takes time to see your answer on the website. The bleeding may have been due to ovulation. If it is 12 days since the IUI, you can do a beta- hcg blood test to determine whether you have conceived or not. All the best!

Question: I had a mis-carriage on 29.11.04 convic.on 28.10.04 2nd convic. on 10.05.05 mis- carriage on 20.06.05. daigon. with toxoplasma IGG AB H25.93 20.12.05 NOW Toxomplasma IGG H11.56 on 10.02.07. then i didn't concieve now i took siphene 50mg for 5days on16.02.07 as per dr.no result.

Answer: Sorry to hear about your miscarriages. I need to know the toxoplasma I gM value, as this would indicate present infection and whether you require any treatment or not. IgG only indicates past infection and does not requirefurther treatment. Are your fallopian tubes normal after the miscarriages ? let me kbnow, so I can advice you further.

Question: Hello Dr., I have been married for more than 3 years now and i am getting the treatment for primary infertility. Case history is that i have pcod, i had cornual blockage of tubes which are now patent after laproscopy, TB-PCR was detected was given anti TB treatment RHEFD for 9 months which has copleted on the 18th feb 07, along with TB treatment from nov 06 i was taking siphene 100mgm and duphastone 10mgm till feb 07, in march i was adviced not to take any madicine my last LMP was on 22nd feb 07, today is 26th march 07 i have not had my periods as yet, in case if i will have my periods my Dr. has adviced me to take tab Letroz 5mgm for 5 days, has adviced me to go for follicular study and when the egg is 17-19mm should take inj. Hucoa 10,000, and from day 16th * 10 days tab dubagest 200 mgm to be kept in vagina for 4 months she has advised me for the above mentioned treatment. want to know if the right proceedure is followed. thanks & regards

Answer: Sorry to hear about your problems. The treatment adviced is fine. If , in spite of ovulating, you do not conceive in 2-3 months, try IUI ( intra- uterine insemination ) .Sometimes, there mayn have been damage to the inner parts of the tube due to the TB, and in that case IVF may help, iff yopu do not conceive in 6 months. ALl the best!

Question: HI DOCTOR.im from india and im 33yrs old, trying to conceive since 3 yrs.my husband's report r fine. in aug 2006 iwas detected with left fallopian tube blockage and anovulatory cycle.my gynec. recommended hormone replcemrnt therapy for 3 months but i didnot take it at all and got pregnant in dec2006 but unfortunately after 2 months ihad missed miscarriage.my doctor asked to follow 'Expectant management'and not MPT or D&C and i got my periodsin abt 20 days and it was vv painful but now everything is cleared up and my bleeding has stopped.i went for TORCH test and IgG titre for Rubella was much higher(144)though i m not vaccinated for it and for cytomegalo(5.94).my doc said its fine because IgM for the 4 viruses was non-reactive,but still prescribed Valcivar-1000 twice daily for 3 weeks.my first Qu. what caused missed miscage Qu.2what is valcivir, should i take this ,it has any side -effects? she has also recommended duphaston for 2 cycles starting from 5th to 25th day shuold i take this also becoz i b

Answer: It is difficult to say why you had a miscarrige- but could be due to chromosaomal anomalies in trhe fetus, infections like toxoplasmosis etc, or a hormonal deficiency. What is the toxoplasma I gM titre ? if this is high, you might need treatment for this. Since Cytomegalovirus I gM is non- reactive, you do not need the valcivar. Duphaston will be fine. Hope you are alos taking folic acid tabltes. All the best!

Question: Hello Doctor, I am trying to get pregnant since 15 months. My pelvic scan was normal and my ovaries were of normal size.Dr gave me Siphene 50 mg for 2 cycles. But no success in getting pregnant. Stopped the medicines and after few months consulted other doctor and have recently been diagnosed with PCO.I wonder if I got the cysts because of Siphene. And my cervical mucus is affected very badly due to Siphene. What I want to ask is Did Siphene cause the cysts on my overies and bad cervical mucus? They were never before there. And my periods are very normal and regular. Thanks.

Answer:  PCO is present in 10-15 % of women. It is likely that you had them from the beginning.Siphene does NOT cause PCO. However, it can thicken the cervical mucus. If that is the case, discuss with your gynec. regarding using injections for ovualtion , rather than siphene. All the best!

Question: Dear Dr. I m 27 and married since 2 1/2 yrs...i have pCOS n don't have kids so far. my gyne told me to have dupheston and its been over 10 days n i still have not had them..do u think there r chances of pregnancy? Will really appreciate..

Answer: You should do a urine test for pregnancy. You have not mentioned whwether you have irregular cycles and whether you are ovulating or not, so I cannot advice you further regarding that.

Question: I am in my 12th week of pregnancy. I have been prescribed allylestrenol when I had some brownish spotting and one drop of red blood with second scan showing reduced perisac bleed, which was seen in the first scan. Is the drug safe for the baby? Any side effects? What other precautions are to be taken? My first baby was normal, and the second was a stillbirth at fullterm. I also have a near continuous white discharge, always seen in the urine. Please help.

Answer: It is safe. is the growth of the baby fine now? What was the cause of the stillbirth? Did you have high blod pressure or diabetes during that pregnancy ? Have a culture and sensitivity test done for the vaginal discharge. If there is any infection, take appropriate treatment. All the best!

Question: I am 42yr trying to concieve since 1yr,whent throught the medical checks like ultrasound, fallopian tubes, hormones test, my husband sperms test they are all normal and clear.my dr suggested me to take siphene from 2 nd day of period and on 16 dayDuphaston but hasent helped me .My periods are normal I bleed for 4/5 days. Please advise to conceive.

Answer: Did you take the duphaston after ovulation ? Were you monitored by ultrasound for ovulation ? Let me know, so I can advice you further.

Question: I have pcos. I have been given clomid to be taken on 3rd day of mc and susten on 16th day of menses.my follicile report on 12 th day was 20mm(only one follicile).and no right follicile.kindly tell me what it is.and when should I ovulate.

Answer: Your follicle has grown well. You must have ovulated on the 13th day. So that is fine. All the best!

Question: Dear Dr. Thanks for the answers you have replied to my questions, but doctor though my hus semens are normal and I am ovalting, tyroid is normal, I am taking clomid 100 mg for 2 cycles from day 2 (my cycle is for 26-28 days) but this time I got my periods after 34 days- till now no improvement. I am very much worried, I am already 38 years and My hus is 40, I want to know whether it is because of my tyroid or my age factor, why I am not conceiving - Dr. please give your suggestion as early as possible - waiting for your reply.

Answer: Are you monitoring your ovulation by ultrasound ? That would indicate your most fertile period. If yoyr tubes are patent, you can try IUI( intra-uterine insemination ). This may increase your chances of conception. ALll the best!

Question: In Jan. 2006 I started my period and it lasted 45 days! My doctor did a vaginal ultrasound, came back that my uterus was enlarged and lining thickened. She then did an endometrial biopsy, which came back normal. So she started me on Provera (stopped the bleeding within 4 hours of taking it) After 4 days on Provera, she switched me to Luestra FE24, but that made me really sick to my stomach and headaches... now I am on my 2nd week of Yaz, feel great on it. I have been successfully dieting (lost 48 lbs.) After I do crunches at the gym, my uterus hurts all day and I have some bleeding. I have now been bleeding with cramps for 3 days and I am only on my 2nd week of Yaz. My doctor suggested Novasure to burn the lining of my uterus (I'm 41 yrs. old) What are your thoughts? Should I take another dose of the provera to stop the bleeding again? Thank you very much for responding.

Answer: First of all, congratulations on your weight loss! however, sometimes a sudden change in the body weight can also cause menstrual irregularities. Are you taking Yaz at the same time everyday. Forgetting it or not taking it at almost the same time can cause some spotting. The pain after crunches maybe abdominal muscle pain, rather than a uterine pain.I think another course of provera would help reduce the endometrial thickness and solve your problems. Do you have any fibroids in the uterus?

Question: Dr am 28yrs old married for 1.5yrs.Mydoc put me on letoval, lynoral for 4mths& siphine for 1 month.but things later i was asked to take LH, TSh test on 2nd day.my lH was 11.6 which my doc is high & asked me to take melmet for 2mths & meprate from 15th to 25th day..and then to take the test again.Could you please tell me if i am on the right track.and how Lh is helpful in this process.Am very eager to have baby.doctor please advice me.

Answer:
Was the LH done on day 2 or 3 of the period ? If not, repeat it, together with FSH and let me know the result, so I can advice you further.

Question: I am 29 yrs old married for 7 yrs. I have PCOD for the past 6 yrs. I concieved after taking Fertyl for two cycles and I had twins who are now 5 yrs old. My problem is I don't get my periods regular and I have been using Meprate every month from 15th day to 22nd day after consulting my Gyney. My Gyney had advised me to take this drug continously every month. My worry is can i use this drug like this and will it have any side effects. Also sonetimes I have lower back ache and abdomen ache and sometimes my two legs also ache. Please advise me. I am not interested in going for another baby.

Answer: PCO is asociated with irregular periods. You can take meprate every month. Usually there are no side- effects unless if you have high blood pressure or high blood sugar. However, that is not a contraceptive. So, if you do not wish to conceive, you can take a low- dose contraceptive pill which would serve the function of contraception and regularising the period.

Question: My mensus was very regular all the while (30 to 31 d cycle) . I have 1 girl age 6 .Now trying for 2nd child. The moment we started to try my mensus went irregular . Jan it was 43 days cycle , Feb was 25 days cycle and march 4 i got the last it was 19 days cycle . I am confused . I bought clear blue ovuulation kit and stared to test my ovuluation from day 5 of until day 20 . But it did not show any ovuluation . But we still had sex 2 days once . But what i am worried is why this happing to me and is it possible i can be pregnant . Hope i am not anovuluation problem .

Answer: Stress can also cause irregular periods, so stop worrying ! You might also be getting irregular periods if you are not ovulating. have an ultrasound done on day 12 of the period. If the follicles are small, you might need ovulation inducing medicines like clomid. Consult your gynec. regarding this. All the best!

Question: Hi, I'm 27 yrs old. Anyways, i've been trying to concieve for about 3 years now. I have one 7yr old, and had a miscarriage in NOV 06 at 11weeks(or earlier). The problem is i track my cycle every month (27days).Luteal phase 14days. My period lasts aboutdays. In 05 i tried to concieve and had sex around and on the day of ovulation but it threw my cycle off. It made me come on 2 days earlier. In 06 with another partner i got pregnant at first try around ovulation but miscarried. I'm back with old partner, and i tried again around ovulation and it threw my cycle off again 2 deays earlier. I had a problem with tubes at first i had an infection (but it was cleared)and thats how i was able to get pregnant in 06. What is making me menstruate early? want to have a baby.

Answer: Is your husband's semen normal? You mentioned that you had an infection in the tubes- but were both your tubes patent ? Have you checked whether you are ovulating or not on ultrasound ? Let me know, so I can advice you further.

I just want to know whether I can conceive during these while taking diane and metformin. or its waste for 3 months.

Answer: You cannot conceive while on diane. So, you should take clomid to help you ovulate. Consult your gynec. regarding this. Continue the metformin.

Question: Hello Doctor, I m 26 yrs old and tying to have a baby. But i have irregular periods from the time of puberty.

Recently we met a Gynecologist and she had advised me Folvit and Glycomet 850mg, and also advised me to have Balanced Diet, Exercise as my weight was on the higher side. My height is 5.1" and my weight was 68kg and now I am 66kgs. From last week i.e., from 21st of March 2007, I m feeling some changes in my body like Low BP, whole body pain, light giddiness, tiredness, mood swings, didn't feel like eating etc. So I visited a Family Physician and she prescribed me the low BP tablet Nosic but it didn't come down and after 3 days I went again for checkup. At that time I had a Urine test and it was negative. She gave the tablets Sinate and Aciloc and the giddiness and vomitting didn't stopped. She asked me to do the test again after 2 weeks.

On 24th March 2007, I found spotting with lower abdominal pain and back pain lightly and it is continuing till date. When I asked my doctor, she told me that I will get my periods and if I have severe pain then contact her or if I don't get periods after 2 weeks then get pregnancy test done.

May I know the exact reason why this is happening to me as I didn't get the answer from my doctor? What care should I take?

Answer: Glycomet can cause the symptoms you have mentioned. Consult your doctor whether you can take the 500 mgm tablet instead of the 850 mgm. one. Are you still spotting? Have you repeated the urine test ? Which city are you in ? Let me know, so I can advice you further.
