Question
My husband and I have been trying to conceive for over a year. Recently,
we started going to a fertility specialist, who after many tests,
concluded she wasn't sure what the problem was....and so she prescribed
serophene (clomid). I am hesitant to take serophene because the one
thing that we know about me that is problematic is that I consistently
have brown spotting for a few days prior to my period. The fertility
specialist suggested this was a sign that my uterine lining was not
becoming thick enough to support pregnancy and was being shed
prematurely. I have done some reading about erophene, and from all that
I have read, it apparently can thin the uterine lining.
So my question is, why would the specialist prescribe serophene?
Does it make sense to take it...if the tests indicate I in fact am
ovulating?
I just would like a second opinion. Can serophene perhaps aid with pre
menstural spotting.
Thanks
Answer
Serophene would help you ovulate in case you are not in some cycles.
Have you checked on ultrasound when you are ovulating? Also, even if you
spontaneously ovulating, serophene would help you form multiple eggs ,
which would increase the chances of conception. Regarding your brown
discharge- do you have any erosion or on the cervix? That could be a
cause. You can try taking duphaston tablets- one tablet twice a day for
10 days after you ovulate ( as seen on ultrasound). Let me know if the
spotting stops. Serophene would cause the uterine lining to thin out in
some cases. Serophene would not aid in the premenstrual spotting, though
duphaston would. What is your husband's semen report? Are your tubes
patent?
Question
I have a vaginal disorder, the doctor say its a severe inflammation, I
have a yellowish on my panty with an odor I want to know what medicines
you recomend.
Answer
Have a vaginal smear and culture and sensitivity test done. This would
determine which type of infection you have- bacterial, fungal or
trichomonial and the antibiotics which would be most effective for the
bacterial infection, if present. Let me know the results, so I can
advice you further.

Answer
RELAX ! You are ovulating, so that is a good sign.Your ovulation and
endometrium are fine. How are your tubes? What is your husband's semen
report ? Let me know, so I can advice you further. What was the extent
of the endometriosis? If you send me the complete laparoscopy report, I
will be able to judge your situation better.
Question
Dear Doctor, Base on my own counting I am 7 weeks pregnant now my last
period was on May 20,2006 my period is regular since I got when I was 10
years old never mis out my period i try last May 23rd the home pregnancy
test and it comes positive 2nd time I went to Clinic and they take my
urine test again and it comes positive again nd again 3rdly I went to
OB-GYNE for the reconfirmation they take my blood test and it comes out
positive again but on my sonogram they found the sac but they can't see
the fetus they told me its to early to hear the hearth beat of the baby
come back after 3 weeks and will check again they gave me o folic acid
tablet and duphoston also because I got little bit spotting the question
is do I have to worry because I didn't hear the hearth beat of my baby?
is this ok or normal I just little bit worried please answer my inquiry
ASAP. I am base in KHASAB - OMAN
Answer
You should repeat the scan within one week of the previous scan .If
there is no heart beat, repeat the blood test. If the level is falling,
it is a missed abortion- the baby is not growing. In that case you would
require a D& C if you do not bleed spontaneously. If the heart beat is
present, that is fine. I will be returning on 12th August. You can phone
me on 24563181 and let me know what the situation is , so I can advice
you further.
Question
I am 17years old and have had a really smelly discharge coming form my
vigina for several years now. I always feel wet down there and it is
really uncomfortable. I awlays have to change undies as it soaks through
and smells. I have had some unprotected sex a few years back. ABout
6months ago I developed one white spot on my vigina lip. I tried
squeezing it but it did nothing. What is wrong down there and what
should I do to treat it? Thanks
Answer
Have a vaginal smear and culture and sensitivity test done. This would
determine which type of infection you have- bacterial, fungal or
trichomonial and the antibiotics which would be most effective for the
bacterial infection, if present. Let me know the results, so I can
advice you further.
You will have to show the "white spot" to a gynec. as it is difficult to
opine on that without seeing it.
Question
Hi Dr., I am 6+ weeks pregnant and have a yeast infection. i have been
having brown creamy discharge, that has now turned a runny pinkish brown
colour. My dr. has recommended canesten. My questions are: 1. can I wait
until I am in my second trimester before I treat myself? 2. Is the yeast
infection causing the bleeding, and where is the blood coming from? 3.
Is Canesten better than Nystatin in the first trimester? 4. Is it safe
to use the applicator in these yeast treatments during pregnancy?
Thank you.
Answer
Canesten is safe during pregnancy, so you can use it now, rather than
allow the infection to increase. I prefer not to use the applicator
during pregnancy. You can insert the pessary with your fingers.
Regarding the pinkish discharge, which is blood- have you done a scan?
Is the fetus fine ? The pink should stop, so you should discuss with
your doctor regarding that.
Question
Hi, I just got off the ortho evra patch about 3 months ago. And my
period has been regular about one month, and now is it irregular. Also,
during when I am almost about to start my period, when I have
intercourse, I will start bleeding during intercourse. Not alot, but
noticable. Is this normal?? And could this be from getting off the ortho
patch?? or could there be a problem with me?
Answer
Have a vaginal examination done. This will reveal whether you have a
cervical erosion or polyp which is giving you this bleeding. Since you
are off the patch , the bleeding should now be regular. if the vaginal
examination is normal, have an ultrasound done which would reveal
whether you have a fibroid or an ovarian cyst, which could cause
intermenstrual bleeding.
Question
Hi i had an icsi done on me after 3 failed IUI i m 28yrs old and have a
problem of endometrics and now i had blood test BHCG done which came to
be negative after 13 days but i have not got my periods till now. the
doc. says to discontinue my crinone gel and has told me to wait for my
periods. but i think i should keep appling gel till i dont get my
period.kindly suggest the reason for negative bhcg. and is it possible
to come to know it can turn positive after few days.my last period was
on 12/6/6 and my eggs were removed on 27/6/6 and inserted on 29/6/6
Answer
The beta hcg blood test should be positive 12 to 15 days after the
embryo transfer. Repeat the test and if negative, you are,
unfortunately, not pregnant. You would not get a period while you are on
the gel. So, if the test is negative, stop the gel, so that you get a
period. I wish you all the best for the future !
Question
Know that this is probably a common question but, this is driving me
insane. I have taken numerous hpt. May 1st was my last period that
lasted only 2 days. I went to the doctor for my annual told her about
the period she did a test and it was negative. I have been having
symptoms, tiredness, sore breast, naseus and of course missed period. i
am thinking i could of gotton pregnant when later on when I ovulated at
the wrong time. I don't know i am so confused. Can someone help. Do i
need a blood test? am I pregnant? what is going on?
Answer
Repeat the urine test. If negative,do a blood test - beta hcg . If this
is negative, take duphaston tablets- one tablet twice a day for 5 days
to get a period.
Question
I am 37 ,10 year marriage,no pregnancy,prolong periods after 3month
,folicle are forming but not repturing?
Answer
When the follicle is 20 mm, you can take injection pregnyl 5000 units
for the follicle to rupture. The best time to conceive would be from
when the follicle is around 15 mm till it ruptures.
Question
Dear Doc, I am 27 years old and I have been trying to get pregnant for
17 months now. I have been to the Doctor for a thorough check up inorder
to detect if anything was wrong. The results showed that I was perfectly
fine.My husband's semens have been tested and the result was positive as
well.I have missed my last month's period.I though i was pregnant so I
took a pregnancy test, but it was negative. I have no idea why i have
missed my period and why I am not getting pregnant when i am perfectly
healthy. What should I do? Could something be wrong with me? thanking
you in advance for your help!
Answer
Are you ovulating? Have you determined that by ultrasound ? That would
indicate your most fertile period. Also, have your tubes been checked?
Let me know, so I can advice you further.

Answer
Do a clear plan test or ultrasound to determine your most fertile
period. This is more reliable than the BBT(temperature). Also, what is
your husband's semen report and have your tubes been checked? Let me
know, so I can advice you further.
Question
Trying get pregnant basa temp hasnt helped need more options
Answer
Do a clear plan test or ultrasound to determine your most fertile
period. This is more reliable than the BBT(temperature). Ultrasound
would be the most reliable method. All the best!
Question
Hello, I just had a chemical abortion last month and also got tested
positive for chamydia. I got treated and then got a normal period. But
then about a week later I started to get a dark brown discharge. I dont
know if it's from all the medication I've taken in the past month, or
can it be something like PID. I don't know if I should be alarmed?
Thanks.
Answer
Do a blood test- beta hcg. This will determine whether the placental
tissue is still present and will rule out an ectopic pregnancy also.
Question
HI, MY QUESTION IS,THAT I HAVE HAD A TOTAL HYSTERECTOMY 6 WEEKS AGO, AND
I DO UNDERSTAND THAT YOU WILL HAVE SOME DISCHARGE AFTER SURGERY. MY
QUESTION IS THAT WHEN WILL IT STOP, THE SMELL IS AWFUL AND IT REALLY
SEEMS TO GET BAD WHAT CAN I DO TO MAKE IT BETTER. SOMETIMES IT SMELLS
LIKE SOMETHING HAS DIE INSIDE OF ME. NEED A RESPONSE, CAN'T TAKE THE
SMELL.
Answer
You must see your doctor to rule out any infection in the vagina or the
suture site ( vaginal vault). You might need antibiotics or vaginal
pessaries.
Question
Mam, My wife is 7-weeks pregnet, and My wife and I flew from bangalore
to sydney last week wednesday. And on sunday after having sex with her
she has seen a 2 or 3 blood drops. When I told this to my doctor, she
gave susten 100 hormon tablets to use daily twice. Could you suggest why
does this happen what do u suggest.
Answer
Susten would help stop the bleeding . However, have a pelvic examination
done to rule out any vaginal or cervical infection or polyp. Also, have
an ultrasound done to ensure that the fetus is growing well. All the
best !
Question
Dear Dr , I have undergone two ICSI procedures , one of which was an
ectopic and my rt tube had to be removed. Now our spermcount has
improved significantly with regards to motility and morphology and so we
want to try IUI. My left tube is open as HSG showed 3 yrs ago . could I
get more info about how it is done in your centre ?
Answer
I am going on leave tonight and will be back on 12th august. You can
phone the clinic on 24563181 and take an appointment for the 10th day of
your period. I will do a scan to determine which side your follicle is
developing and when to do an IUI.
Question
I am 38 years old. Last month my period started about 4 days early which
is not unusual to start early. The first two days I had pretty heavy
bleeding and then I continued through my week of my period. However, I
never fully stopped. I continued to bleed lightly. I had a couple days
where there was no bleeding and then it continued again. I started
taking black cohosh three times a day. I was still bleeding lightly and
then had about a week where it was nothing or a little brown. The
following month I started to bleed quite heavy and just assumed it was
the start of my next period for that month. I would like to take an otc
prod or natural remedy before having to resort to my gyn. Is this black
cohosh helping? Should I be also taking something else with it or
differnt?
Answer
I think you need to do an ultrasound to determine whether you have an
ovarian cyst or a fibroid which can cause menstrual irregularities. I am
not aware how black cohosh will help you in your menstruation problem.
Question
I took duphaston tab. for 7 days. i took the last pill on 26/6.after
stopping the pills 11 days are over but i didn't get periods. Am I
pregnant?
Answer
You should get a period within 10 days of stopping the duphaston. You
should do a urine test like clear blue to check whether you are
pregnant.
Question
I am 38 years old having an ivf and I have a diabetes/ my doctor
prescribe me some prednison and I heard that prednison is not good for
high blood sugar. Does prednison will not affect my getting pregnant?
Answer
Prednisolone may increase the blood pressure. Why was the prednisolone
prescribed ? it will not affect your chances of conception.
Question
Hi Doctor, My wife underwent Hysteroscopy for removal of Cyst one month
back on 15 May..Doctor has advicsed CLOME 4th day after period and
DUPHASTON 14 day after period.. What are these medicine meant for?
Please help
Thanks
Siddhartha
Answer
Was a laparoscopy done or a hysteroscopy? A cyst is usually in the ovary
and is removed via laparoscopy. Clomid will help your wife ovulate and
duphaston would help in maintaining a pregnancy.
Question
Hi doctor, I've searched and searched for answers, and I just can't seem
to find any! I just recently started using the Ortho Evra Birth Control
Patch. I was told to start using the patch the first day I got my period
last month, which would have been the 19th. I normally get my period in
the morning or atleast before noon. So when I woke up that morning and
didn't have my period yet, I went ahead and put the patch on. Well, I
never got my period for that week. I got scared and took a few preg
tests, but they all came back negative. About a week and a half later I
start to get this dark brown discharge. Its still going on and I'm not
sure what it is. I did some research and I found that the dark brown is
typically "old blood". Could this be my "period"? Next Monday the 10th
is the beginning of my "off week". Will I get my period then? And will
it flush all of this stupid brown junk out? I got really frustrated with
it the other day and I douched, hoping it would clean it all out... well
it didn't work. It helped alittle I think for awhile but now its back to
pretty much normal. What's going on with me? Will this discharge stop?
Is it because of my bc? Is there anything I can do to make it stop?
Please help! I need answers! Thank you for your time.
Answer
During the first few cycles there might be intermenstrual spotting while
taking the pills. If the problem persists you can consider changing the
pill. Also, if you do not take the pill at the same time everyday, you
might bleed. I presume a vaginal examination has been done and there is
no vaginal or cervical infection.
Question
Doctor u were on leave in April when i wanted to see you. i have come
back from India , but my problem is to clarify that my Doctor in Muscat
coludnt see me pregnant in Scan on 22nd Feb and then told me that i have
PCOD. She prescribed me Primolut-N twice a day for 3 days to get my
periods. I was so sure that i am pregnant coz of the heaviness and
tiredness in my body. After taking the pills on 7th march she told me i
am preganant of 8 weeks. She then started giving me Pregnyl injections
to sustain the pregnancy twice in a week for 4 weeks. The real trauma
was on 2nd April when the doctor detected 3.2 mm of Nuchal Translucency
on the back of fetus' neck.
Now I am back from India with all the perfect reports and
aminiocinthesis done and am 25 weeks pregnant. my question is that was
that increased NT coz of the pills and injections that the doctor gave
me? and how is it that i concieved when a week before only the doc told
me that i have PCOD and its difficult to con! cieve? Did the pills help
me to conceive?
Thanks Payal
Answer
Take an appointment by phone- 24563181. If you are already my patient,
bring your file number when you come. . However, I am going on leave
tonight and will be back on 11th August. Take an appointment after that
( my receptionist will be at the clinic). Please bring all your reports.
The nuchal thickness maybe increased even if the chromosomes are normal
, when there are certain other conditions. The medication does NOT
increase the nuchal thickness. The pills do not help conceive either.
You must have had a delayed ovulation and conception( which is common
with PCOD) and that is the reason the test was initially negative.
Please bring all your reports when you come to see me.
Question
Dear docter I am 27 years old having 4 years baby, here i am writing my
ultrasound report-ultrasonogram of lower abdomen(TVS) The urinary
bladder appears normal. The study show normal sized uterus with uniform
mymetrial echoes. The endoemtrium appears normal. both ovaries are
enlarged showing multiple smallcystic areas in the periphery. the right
ovary is measuring about 40X26X20mms which is equal to right 10cc of
volume. The ovary is measuring about 43X24X20mms which is equal to about
8cc of volume. No pelvic mass is seen.
IMPRESSION:-The findings are in favour of PCOD plese give me your
advice as soon as possible
Answer
PCOD is a condition present in 5-20 % of women. You must have had it
even when you conceived earlier. If you wish to conceive now, so a clear
plan test to determine when you are ovulating. Also, are your periods
regular ?

Answer
Your question goes to the omanaccess site and is then sent to me . I
answer the questions and send the answers to that site and then they
send you the answer. I do not know why you did not get the answer. You
can write to the website to clarify that. However, if your tubes are
patent , you can try IUI with superovulation. Let me know which city you
are in , so I can advice you further.

TRIQUILAR First registrant to get period and from 5th day onwords use
the above tablet. No wi want to Conceive. So is these tablets helpful or

Answer
Your question goes to the omanaccess site and is then sent to me . I
answer the questions and send the answers to that site and then they
send you the answer. I do not know why you did not get the answer. You
can write to the website to clarify that. Triquilar is an oral
contraceptive pill and will not help you conceive, only to regularise
the periods. You must stop the triquilar and then check whether you are
ovulating or not. This can be done by the clear plan test. Let me know
the result, so I can advice you further.
Question
I had an abortion a month before I got my tubes tied, is it normal to
have viginal bleeding after an tubal ligation and when should does yo?
Answer
Your question is " I had an abortion a month before I got my tubes tied,
is it normal to have vaginal bleeding after an tubal ligation and when
should does you." I do not understand the last part of your question.
Please clarify. Regarding bleeding, you can have some bleeding off and
on after an abortion, which should settle within 6 weeks. If the problem
persists, see your gynec.
