Hi Doctor, I am 37 years old. I have FSH level of 41 and advised that I am going through early menopause. My ovaries are small and today the doctor did an ultrasound and found no basal follicles in my ovaries. I am on HRT therapy. The doctor has advised me I have no chance of having a child anymore. Is there any chance of my having a baby at all? I am very depressed. Please advise.

Answer

If your FSH is repeatedly high, your ovaries are not capable of forming "eggs ", so it is not possible for you to conceive , unless you do IVF with "donor" eggs. Was your FSH high on several occasions before starting your HRT? Had you stopped your periods or were they irregular? A few women have a high FSH on some occasions , but are menstruating regularly and forming eggs. If only one FSH has been done and you are very keen to conceive, I suggest you stop the HRT for one month and repeat the FSH.In the meantime, let me know the details of whatever I have asked ,so I can advice you further.

Answer

Have an ultrasound done to ensure that you do not have any ovarian cyst . If you have not had a scan before, it will also ensure that the pregnancy is growing well and is within the uterus.

Answer

The report should mention the size of the uterus, whether there are any fibroids in it, the size of the ovaries, whether there are any cysts in them and whether the ovaries are polycystic in appearance. You should ask for a detailed report. "Unremarkable" probably means that there are no abnormalities !

Hello Doc, I have regular periods, but before the actual normal flow, there is atleast 2-3 days of light spotting which is followed by 4-5 days of normal menstrual flow, is this normal. Iam trying to get pregnant and I would like to know, which day to start counting as my first day of menstruration .... the day I start spotting or the day I actually start bleeding because these are 2-3 days apart. please advice. Do you have a cervical erosion or a polyp? This can give rise to spotting premenstrually. If you do not, you do not require any further treatment as that then is your cycle pattern. You can count your first day of spotting as your first day. However, rather than rely on dates for conceiving,do a clear plan test to determine your most fertile period. All the best !

Can Duphaston prevent misscarriage because my other half want for a test and it was found out that the membrene and the womb are not joined together, so she was giving Duphaston

Duphaston does help in preventing a miscarriage in some cases.

Dear Doctor, Thanks for ur reply.Actually I had laproscopy before the laprotomy.During the laproscopy doctors found out severe endo and addhessions all over and at that time it was treated with lesar.there were cysts in both overis and were drained I also had both tubes bloked. unfortunatly after the laproscopy I started internal bleeding and have to had laprotomy to stop the bleeding with 24 hrs.since then dr did'nt priscribed any medicine and asked me to have HSG test in may which showed that the right tube is open and only the left one is bloked.my serum progestrone (day 21) showed the result as 45. I had the laproscopy on 30th nov-05 and laprotomy on next day. Now I am very confused and don'nt now what to do becouse before the HSG dr was thinking about ivf.but now I could'nt get a chance to see the doctor.IS it ok for me to wait and see if I can get prgnant spontaniously or move on for medical assistence. I am very disperate for answer .plz help and plz reply asap.thanks

Since you have many adhesions, and the laparotomy has found both tubes are blocked, you would require IVF. You can check with your doctor whether they found both tubes blocked at laparotomy . If one tube was patent, you can try naturally ( without IVF ) for a few months. Let me know the complete laparotomy report, so I can advice you further.

Answer

Your next period should be around 30-45 days after the miscarriage. In case you are planning to conceive now, it is advisable to wait for 2 months after the miscarriage before you conceive again.

You should be seen by your obstetrician as these "bumps " maybe granulation tissue which would require further treatment.

Dear Doc, I am diagnosed to be mildly PCOS and hypothyroid and since I would like to concieve my doctor prescribed me Metmorfin 1500 mg per day...After using this dosage I did ovulate (according to pee test at home) and now I am wondering if I should stop my metmorfin until i get my next peroid and then start again or continue and if I get pregnant will it harm the baby? Some recent studies have shown that metformin is safe in pregnancy and can help prevent a miscarriage in certain cases of PCO.If you have had a pat history of miscarriages, you can continue the metformin. Otherwise , you can stop it . Did you do the clear plan test ?Certain drugs interfere with the test , so it may not be absolutely reliable. Ultrasound would be more reliable, as it would show you the actual size of the follicle.

Hello Dr. I am a guy, and I have somewhat a problem with my penis. I have these really small yellow-ish bumps on my foreskin. Some of them gather up, some of them are just alone. What is this? The bumps are about as small as a dot (.) and they don't itch, smell, or anything, they're just there. Is it normal? You should see a doctor who can examine you and decide on the treatment required.

You have mentioned "proflien ". I presume you meant " prolifen ". Prolifen is clomiphene citrate which helps the eggs to form. If taken in large doses and without ultrasound monitoring, there can be enlargement of the ovaries and formation of cysts. Also, there is a risk of multiple pregnancy. Unfortunately, no treatment is

I am a 29 year old woman recently married before four months.i had painful menstruation,clots sometimes ,my husband was sexually weak so we consulted a male gync he advised some medicines and harmone injections to him and me also. my menstruation was on 14th april last month but till today that is 16th may I have not got menstruation. could it be the effect of harmones or have I conceived, I have no particular signs except swelling breasts which was occuring before 4 6 days of menstruation when I was un married. First of all - RELAX ! Even if there is social pressure, things will be worse if you are tense. If you have not yet got a period, do a urine test for pregnancy by the clear blue test. If positive- great ! If negative- I need to know what medicines were given to both of you, so I can advice you further.

Do you have PCOS - polycystic syndrome ? What is the result of your serum FSH, LH and prolactin on day 2 of the cycle? Are you ovulating ? This can be determined by the clear plan test or by ultrasound. What medicines did you take and in what dose? Were you monitored on ultrasound during that time ? What was the size of the follicles on day 14 of the period? let me know all this, so I can advice your further.

I am 33 years old. I always had normal periods since I was 13 years old. They come monthly every month on the same day 15th - 20th. Until I had my youngest son (born in 1996). And then all of the monthlys where very CRAZY! Have a period then miss a periond. One month light, one month flooding. I told my gynecologist. And he recommended that I lose weight. I gained alot with my third child. For years I kept the weight on. However 2 years ago I lost 72 lbs. And after that I was back on track. Normal monthly's. With the exception I suffer with lower stomach cramps, lower back ache, tender sore breast,and I am so moody. Either I am so sad and depressed with crying spells or I am so ill and short tempered. I do not like my self like that. And all of that has started in the past year to year and a halff (12-18 months ago). And a year ago this month or last month (April or May 2005) I had a REALLY heavy bad ODD period like I have never had before. I started without any ! warning. (small irrability) And when I did it flooded and had huge blood clots. (golf ball size). I did tell my gyneo. And she said that it was normal. So since then I have still had periods monthly maybe a day or to late and some months light in flow. and other months very heavy the first day. But still with the moodiness,or depression. And I started this morning for May of 2006. And It was flooding like it did a year ago. I had a tampon in and it still flooded out and down both legs. I went to my shower and cleaned up. And blood just flowed and many large blood clots keep coming out. I want to know why do I seem to do this once a year? And why do I now for 18 months suffer with ill, mood swings,depression the week before I start? My husband and kids and myself know when I am going to start because of my mood swings. What is all of this. How do I treat it. And why has all of this just started? Thank you. Please do not post my name on this question article!

Your question goes to omanaccess site and is then sent to me ( without your name ) , so we respect your privacy, so don't worry about that. The mood changes you have are part of the PMS- premenstrual syndrome and many women suffer from that. You may now be stressed with the kids and housework etc, so the mood swings maybe more marked than earlier. Are you on the pill? Some women have more mood swings while on the pill. Unfortunately , there is no real cure for the mood changes. You can try a multivitamin tablet for a few months. If that does not help, see your gynec. and discuss regarding buspar or any other drug to be used for a few months. I must warn you, however, that no remedy will really work. You have to come to grips with yourself and try and keep yourself mentally occupied , so that your mood changes do not affect your daily life. Regarding your bleeding, do you have a fibroid in the uterus or an ovarian cyst ? Have an ultrasound done to determine that and let me know. All the best !

Hi Doctor, I have been having repeated vaginal infection with a thick discharge for the past 8 months. The Culture and sensitivity tests suggests E. Coli. My doctor has given me medications for fungas and bacteria. (Fluconazole and Tinidazole and Lactobasicillas and Clingen/Veperaz). This reduces the problem when I take the medicines but recurs within 15days. what can be done about this? Are there specific methods to handle this problem.

Were you and your partner simultaneously treated with tinidazole ? Repeat the vaginal smear and a culture and sensitivity of the vaginal discharge and let me know the complete reports, so I can advice you further. Also, what is the ingredient in clingen ?

I am going through the procedure of IUI. Its been performed 2 times. Botth the times I was asked to keep the cyclogest passery until I start my periods. Both times, after keeping it for 7 to 8 days, I felt burning sensation in passing urine. Also I had severe pain inside vagina. I also have the feeling that my bladder is not completely emptied when I am urinating. And urine keeps coming bit by bit for 10 to 15 minutes at the time of passing urine. Last month when it was my first IUI, after 2 3 days of my periods this feeling was over but his time again as I am having my second periods it is still there but it is becoming milder day by day. It never happened before in tthe past. Is it because of the Cyclogest? I drink very less water but never had this problem before.

Have a urine test done - routine & microscopic. This will determine whether you have any urinary infection which would account for the burning while passing urine. You must drink plenty of fluids. The burning in the vagina could be due to the cyclogest. You can discuss with your gynec. regarding an alternative treatment. If you are in Oman, you can see me. My wife is pregnant and is completing her 7th month, she has been a diabetic patient for a while, i mean 4 years, she had also gone for mtp and this is our third child, first is a baby bosy second mtp and third now carrying with 7th month under completion. Her sugar level is not comming down and now she has reached taking 100units of insulin on daily basis, recently her doctor gyanec had refered us to go to XYZ endocrinologist a very famous in hyderabad to see his expertise on the sugar and he had advised us to start glycomet, twice a day along with inuslin, he said the formation of baby is now completed and now e can go for tablets, i would like to know will there be any problems using this tablet or not during 7th to 8th month, please suggest in this reagrds.

Which city are you in ? Let me know, so I can advice you further regarding the medication. You can use oral tablets. However, if you are taking insulin once a day - you should be taking it twice a day for adequate blood sugar control. I , personally, feel that the right type of insulin in a correct dose will give a better blood sugar control than tablets.

Dear Doctor, I am 26 years old, got married for 1 year. Before 6 months did laparoscopy to remove chocolate cyst in my left ovary. I am trying to get pregnant for 5 months. But I couldn't and went for a check up. The Ultrasound scan result shows that again chocolate cyst is developing in the same ovary. Hormone levels, fallopian tubes(no blocks), uterus are normal. My husband's semen analysis report is also normal. Now the size of the cyst is nearly 5.3 cm in left ovary. Last month when I went for follicle study, in right ovary found two follicles which ruptured on 13th day. My doctor asked me to take a multivitamin tablet which contains folic acid and also Uterogestan 100 mg per day. Is there any chance for me to get pregnant? If I get negative result, do I need laparoscopy again to remove the cyst? Will it reduce the chance to get pregnant or will it affect my health by any chance?

Unfortunately, chocolate cysts do recur. If the cyst is not increasing in size and you are ovulating, you do not need to do surgery again for the cyst. If you do not conceive in 1-2 months, try IUI as it may help. All the best !

I was dignosed with PCO. I was taking 1500 mg of metformin. I am also insulin-resistant. My question is will this help to relieve my acne and hirsuitism? If yes, then how long do I have to take the medicine before I see some improvement. The acne and hair problem is really ruining my self-esteem. Can I take something ot do something to get rid of these symptoms?

PCO is accompanied by hirsutism ,acne, obesity, irregular periods and anovualtion. Are you planning to conceive? Metformin would help you to regularise your periods, ovulate and increase your chances of conception. It will not have much effect on the acne and hirsutism. Also, whatever effect is seen will be temporary and thwe problems would recur on stopping the drug. You can do electrolysis for the hirsutism and use an anti-acne preparation like exfoliac soap and any other acne preparation which suits you. You should not get upset with these symptoms as 10-20 % of women have PCO and it is a condition which remains .

Hello, My name is sharon. About a month ago I had a hysterscopic procedue done. I haven't had my cycle this month, it's only a few days late but I am having the symptoms that is coming.but nothing has happened. Today I went to the restroom and there is a bright yellow, greenish yellow discharge at the top part of pantyliner and towards the middle part of the pantyliner I have a brownish discharge. I don't know whats going on. Can the hysterscopic exam affect your menstral cycle. I am due to have the uterine lining cut away are there any seerious side affects from that procedure.

Please let me know what was done during the hysteroscopy. Was it only for diagnosis or were any polyps removed ? I presume you are going to have an endometrial ablation done for excessive bleeding. Do let me know , so I can advice you further. The following is some information regarding the procedures:

  • "Endometrial ablation. Using ultrasonic energy, your doctor permanently destroys the entire lining of your uterus (endometrium). After endometrial ablation, most women have normal menstrual flow. However, some women have little or no menstrual flow after the procedure. Endometrial ablation negatively affects your ability to become pregnant.
     
  • Endometrial resection. This surgical procedure uses an electrosurgical wire loop to remove the lining of the uterus. Both endometrial ablation and endometrial resection benefit women who have very heavy menstrual bleeding but don't have other underlying uterine problems such as large fibroids, polyps or cancer. Like endometrial ablation, this procedure negatively affects your ability to become pregnant. "
     
  • There is a risk of perforation of the uterus with damage to the internal organs like bowel during the procedure.

Hello Doctor, I'm 25 years old.I have polysystic ovaries and irregular periods from the very start.I conceived my first baby by taking cerophene and primarene.Now my son is 15 months old,i'm taking no conteraceptive but still not getting pregnant.Should i go for the cerophene again.Is it the right time to plan another baby?

If you have had a vaginal delivery, you can conceive now. Are you breast feeding ? If you are that might be causing the irregular periods and preventing ovulation. If you have stopped lactating, do a clear plan test to determine whether you are ovulating or not. If you are not, you should take the serophene. However, ultrasound monitoring is essential while taking that. All the best !

As per asked by u my fasting level in lab is 29 Doc I wanted to ask u that my endocrinology doctor had told me to stop the medicine Ginette 35 as it will gain weight so kindly recommend me some medicine to avoid PCOD (that should be available in India) and as per ur last time answer u told me to do CLEAR PLAN TEST at home please tell me what is it? and how can I do it? is it available in medical store? as im in India is it available here? This time TSH increased till 12 normal range is 0 to 4.4 and doc told to take ELTROXIN 1 tab daily ( first it was ½) Now from this month doc have stopped GINETTE 35 And given me SIPHENE I have already completed the course is it possible that I can get pregnant ( cause I have PCOD Prablem) please give me the solution as this mails answer will get in 2 weeks and I will get my period around 26th of this month so if u can suggest me any medicine for conceiving pls do so. Since you are planning to conceive, you have to stop the ginette 35. The clear plan test is a test which detects the rise in LH hormone just prior to ovulation. It should be available in medical stores. If not, have an ultrasound done on day 12 of the period to determine whether you are ovulating or not. Losing weight is an extremely difficult task , but do your best with a combination of diet and excercise and I am sure you will lose weight ! That will help you ovulate also. Did you ovulate with siphene ? You can conceive with the right treatment even if you have PCO , so don't worry ! Let me know where you stay in Mumbai, so I can advice you regarding a doctor near your house. If you do get a period take siphene in a higher dose if you had not ovualted before. All the best !

When you get a period in June, you should start the "pill " from the first day of the period. You can use Yasmin or gynera or gracial. If none of these are available, ask the pharmacist to go through the "martindale book " to find similar drugs available in India. The :pills " I have mentioned are the low- dose pills which cause minimum side effects. All the best!
