Hi, I am 53 years of age this year. Over the past four years I have had a lot of instances with heavy periods. Have had two hystercopy's and a DC which showed fibroids. The first hystercopy was told no fibroids and put on primolut. After a while I came off this and managed my periods by myself but was eventually forced back on this as the heavy periods returned and I became anaemic. I am now back on primolut and have been taking one a day and do get occasional break through bleeding. My specialist says that a ysterectomy should be considered. I think I am nearing menopause and at my age shouldn't need one. Is there any other procedures I could investigate which could give me a few years grace before menopause or should I just stay on primolut?

What are the sizes of the fibroids? You can try cycokapron or dicynone from the second day of the period -three times a day for 3 days to reduce your flow. These are not hormones, so they are safer than primolut when taken for many cycles. Take iron tablets if you are anaemic. Also, what is the thickness of your endometrium ( uterine lining) ? This is best determined by vaginal ultrasound just after a period.

But cannot fall pregnant, I have had ultrasounds and blood test's to see if i am fine and everything came back clear except and enlarged ovary and liver and some cysts on my ovaries. Do you have PCO ( polycystic ovaries) ? Are your periods regyular? What is your husband's semen report? Do a clear plan test to determine when you are ovulating and this will determine your fertile period.

Regarding your dicharge- you can use betadine vaginal pessaries for 7 days . Do you have PCO( polycystic ovaries) ? Are you ovulating ? This can be determined by a clear plan test or by ultrasound. If you lie down for around 2 hours after intercourse, it gives enough time for the sperms to go into the uterus. It does not matter if the rest of the semen comes out after that ( as it does with everyone ). What is your husband's semen report? Let me know all these , so I can advice you further.

Dear Doctor, I am married for 4 years. I had 1 miscarriage and the second an abortion. I have PCO. The first miscarraiage in 2003 was at a few weeks while the second one was an abortion in oct 2006 at 4 months.During the secnd pregnancy, I had off and on bleeding. The docs said that abortion was needed as there was no fluid and severe ohgohydrammos. Due to PCO my periods are irregular and are missed for a span of 3-4 months. For eg my lmp was in Jan 06. The foll tests have been conducted :ANA, result positive+1, FSH-result 4.28, LH-2.70,Prolactin-13.59, T3-95.60, T4-9.40,TSH-1.86 & Toxo- negative 0.35. My doc has advised me to continue taking Metformin(for 3-6mths) along with Duphaston(for 5 days) and then check if the period occurs. Please let me know what are my chances of having a healthy pregnancy. P.N. the earlier two pregnancies occured without ovulation. Are the medications that i am taking ok and is it possible that i will have a healthy pregnancy this time! Please advice?

Relax! Except for the ANA which is positive, your other investigations seem normal. Do let me know whether the T3, T4 & TSH are in the normal range ( as these may vary in different labs). If you are in Oman , you can take an appointment with me , ( 24563181) and I can advice you from the beginning, so that you do not have another miscarriage. You have mentioned " earlier pregnancies occured without ovulation" - that is not possible. Did you mean something else ? Are you ovulating? This can be determined by the clear plan test.

Relax! Many women have these problems, so there is nothing to be ashamed about. What is your testosterone level and do you have irregular periods? Let me know, so I can advice you further.

Hi, I am 30yr and now 8weeks pregnant. I am suffering from a ailment called cuteaneous vasculitis. My doc adviced me to go through a series of blood test.All my blood test were-ve, but for ANA which is +ve.After consuting many doctors she adviced me to be on rest for 3moths and try to stop straining which triggers vasculitis. Now she has also adviced me to take Ecosprin 75 every day so that my ANA+ should not effect the babies blood. I checked through many sites which talks of side effect of Ecosprin during pregnancy. Kindly let me know if it is ok for me to be on that medication?

Ecosprin is a low dose aspirin and is safe during pregnancy. You have been adviced the correct treatment- I give it to my patients also, -so stop worrying about it ! All the best !

You cannot take gynera depending on when your husband is in town ! You have to take it throughout the cycle for it to be effective. If you have not been counselled about it and wish to know all the details, you can take an appointment with me and I can advice you further. ( My no. is 24563181 ). I am 36 years old I had my uterus romoved in 1999. Lately I have been having lower pain in my left ovary area and have had a white discharge with a bad awful oder and also have had abdominal swelling and my breast have been very swollen and tender and are excreting liquid as if I were pregnant can you please offer me some suggestions?

First of all, have an ultrasound done to determine that there are no ovarian cysts. have your prolactin hormone checked, as if this is high, you may have secretion from the breast. Regarding your vaginal discharge use polygynax vaginal pessaries for 6 nights. If the problem persisits, have a vaginal smear and vaginal swab for culture & sensitivity done to determine the type of inection you have.

During my preganancy there was a simple cyst growing in my ovary which the doctor had asked me to get operated which had grown to 12cm . After my delivery the cyst reduced to 6cm but the docter had adivised me to keep a check and get a sonography after every 2 months. 3 weeks ago I started bleeding after which i went for a sonography and found that is after 8 months after my delivery it had grown 7 cm in size and it was an endometric cyst now. Doctors advised me to get it operated. I got it operated last weeks the doctor had advised me remove the copperT and go on contraceptive pills. She suggested that these pills will help me with unwanted preganacy and also willnot allow the cyst to grow in furture. I have a query that will it really help me ? As I am not very keen to use contraceptive pills. Doctor please advise?

Sorry to hear about your problem. Yes, the contraceptive pill would reduce the size of the cyst or even prevent it from growing very large. This is because the hormones in the pill would reduce the growth of the cyst. However, you would still need to be monitored by ultrasound every 2-4 months.

You are ovulating and your husband's semen also seems fine. Have your tubes checked.This can be done by a hysterosalpingography ( which does not require hospitalisation ).Also, has a PCT been done ? Let me know the results, so I can advice you further.

I am 32 and married for 8 years, I was diagnised with endometriosis. Recently I have undergone an operative laproscopy and endometriosis scars were removed. Afterthat my gynec told me to take clomid 100 mg from second day for 5 days and gonal f 75IU on the second day. I had experienced severe ovulation pain on 12th day time and we had intercourse during these days. But from 16th day onwards I am feeling severe breast tenderness and muscle aches and mood swings. Today is my 20th day. Why I am feeling all these so early..normally PMS will start before 3 to 4 days. Is that beacuse I was taking these medicines? Will I get pregnent this time? Once you ovulate, you may get breast tenderness and mood swings. But, you can be pregnant.How many follicles did you form and did they rupture? All the best !

Progesterone treatment for PMS is only required in very severe cases and is not given in all cases. Insulin stores magnesium. Conversely, magnesium is necessary for the action of insulin and the manufacture of insulin. If your cells become resistant to insulin, you can't store magnesium so you lose it through urination. Studies have shown an association between insulin resistance and magnesium deficiency. Magnesium deficiency is relatively common in diabetics. Insulin resistance is common in PCOS , therefore there maybe deficiency of magnesium. You can take a multivitamin mineral combination containing magnesium.

Dear Doctor, I am 29 years,married on 10thDec 2004.I had a miscarriage on 24th Mar 2005,hardly two months after conception.Durin this pregnancy,the ultra sound report showed fetus is 4-5 weeks whereas by date it was supposed to be 7 weeks.Since the miscarriage I had been had been trying for a baby,I had undergone Laprascopy with heteroscopyoperation on 1st Sept 2005 for coagulating endometrial spots which the doctor said that it was the reason for the miscarriage.After the operation my reports says I'm absolutely normal and I can concieved naturally.Till now I'm still trying for a baby and the doctor has prescribed Ecosprin 150mg,14 tablets(half a day)that is for 28 days and also Clofert 50 mg.My worry is that wheter Ecosprin 150 mg is a relevant medicine for my problem and is it safe or does it have any side effect.Is there any major problem about why the fetus could not grow.

The fetus may not have grown due to various reasons like chromosomal abnormalities, hormonal deficiency, infections like rubella or toxoplasmosis or auto-immune diseases. Ecosprin 75 mgm is safe during pregnancy and can help in preventing another miscarriage . I also give some progesterone supplementaition.You will have to check with your doctor regarding that. I am 37 years old, last period (27.02.06) I have been on clomid 100mg on 5th day of my cycle for 5 days & on my 13th day of my cycle I have been given hcg injection. Today it is 27.03.06, for the past 6 days my nipples are paining & I am getting muscous & cervix is soft & high. Check your urine for pregnancy. Sometimes there can be bleeding during pregnancy. If positive and you are 6 weeks pregnant, have an ultrasound done to determine how your fetus is. If you have not conceived, have a PCT done to determine whether the sperms are dying in the cervix and then you may require IUI( intra-uterine insemination ).

I hope you are well. Please allow me to explain my concern. My wife & I are trying for a baby. Her blood tests by her gyno. have revealed that her Prolactin is very high. About 3 tests have been done & only the most recent one showed that it was normal. So her gyno. has suggested to us to get an MRI done for her brain w/contrast & consult an endocrinologist.

My question is, should I consult the endocrinologist before the MRI? Will he also ask to get the MRI done?

Also, my wife says that she thinks the 2 blood tests done during her 2 seperate periods showed high prolactin while the last one showed normal prolactin because it was done after her period. Is this possible?

Has any treatment been given for the high prolactin ? If not, what were the values? Prolactin can increase due to stress and around ovulation in some cases, so that would explain a normal prolactin just after a period. Let me know all the results of the prolactin, so I can advice you further.regarding your MRI. When the prolactin is high, there might be a small tumour in the brain ( NOT malignant) which gives the high prolactin.

Hello , Doc.Currently I live in Japan.I have a cycle of around 32-34days. We have been trying for baby for around last 12-14 months but not succeeded. I met a Doc in indai july last year, she gave me Natvie 400, B-LONG and duphaston and fertomid but somehow I didn't conceive.Last month i.e Feburary I again tried the medicine. She told me that if your mensuration doen't start after stooping duphaston ,it could be the sign of pregnacy .I took last Duphaston two days back and has not got my cycle. Does that mean I could be pregnant .Today is the 33rd day.

You would get a period around 5-10 days after stopping the duphaston. Check the urine after that to determine whether you are pregnant.. What dose of fertomid are you taking? You must do at least one ultrasound around day 11 of the period. This will determine whether you are ovulating or not with the given dose of fertomid and also that your ovaries are not forming cysts with that. All the best !

What is your husband's semen count? Are you being monitored while taking the clomid? What is the size of the follicle? Has a PCT (post- coital test ) been done? Is it normal. Let me know all this, so I can advice you further.

Hi, My doctor put me on this medication called medroxyprogesterone 10mg for 10 days.I started taking this on the 15th of March and I ended on the 24th.What I wanted to know is when will i get my period? You should get a period within 7-10 days of stopping the tablets. If not, do a urine thest for pregnancy. Are you ovulating? This can be checked by a clear plan test. i usually give duphaston tablets ( instead of medroxyprogesterone) once you have ovulated to increase the chances of conception.

Relax ! A high Prolactin level can give rise to iregular periods. Have you been given any medication for that? Also, what is your hemoglobin? Let me know, so I can advice you further.

Have an ultrasound done on day 5 of the period to determine whether the cyst has gone or not. Are you ovulating ? This can be determined by the clear plan test. What is your husband's semen report? Since your pain during intercourse is rare, you do not need anything for that. However, if you have any vaginal secretion - ( white or yellow discharge), let me knowthat also, so I can advice you further.

I am sure you will conceive! You have not mentioned whether your tubes have been checked or not. How many eggs are you forming now? The chances of conception increase when you have 2-3 eggs. Let me know these, so I can advice you further. You do not need to worry about the pain, just take care to avoid those positions.

Angie, your question goes to the omanaccess site and then comes to me and I send the answers back to them , so there may have been a delay in getting the answer. You have to let me know your age, whether you have had other symptoms of the fibroid in the past like severe bleeding and pain. What is happenning about your beta- hcg hormone ? Has it reduced? If yes, then the pregnancy is not growing, so you need not worry about an ectopic pregnancy.

You probably have PCOS (Polycystic ovarian syndrome ). This is associated with a high LH:FSH ratio ( more than 3: 1 - as in your case) , irregular periods , excessive hair growth and obesity. Regarding the thyroid levels, check whether they are in the normal range for your lab. if not, let me know the results again together with the range, so I can advice you further. How frequently are you taking modus? You can try duphaston tablets- one tablet twice a day for 5 days to get a period when it is delayed. This may cause less depression for you. However, all hormones may cause some change, so do not worry about it.

I have a history of ovarian cysts, and endometriosis. I have had 4 surgeries for endometriosis with the last being in late 2002 because my ovarian cysts were large and micro-surgery to remove endo. The past few months after sex with my husband I have been bleeding slightly and it is just about 1 day and light. We had sex the other night and now since then I have been bleeding like I am having a period but with much less cramps, just very mild ones that come and go. The bleeding has me scared though because its quite heavy for mid-cycle. It has small clots in it to. Could it be that an ovarian cyst ruptured? Im not sure because I would think that would have caused more pain. I just had a normal pap smear in Dec. 2005.

Sorry to hear about your problem. Unfortunately, endometriotic cysts tend to recur.Ovarian cyst rupture would rarely cause bleeding after intercourse.This could be due to a cervical erosion or a polyp. However, have an ultrasound done to determine the size of the cysts and whether there have been some other changes within it.

Hello doctor, I am suffering with irregular periods. I got married 1-1/2 years ago, but did not get pregnant in this 1-1/2 years. When I was diagnosed a year ago, was found to have PCOS, but no thyroid disorder. The prolactin level was high and it was treated with half a tablet of Cabergoline 0.5-mg tablets x 6 weeks. Now I am asked to do follicular study. The size of my egg on the 18th day is about 16 x 15 mm and endometrial thickness is about 8.5 mm. Please let me know the normal size of the egg and endometrial thickness during this time and what steps can be added further to the treatment. I would be thankful to you if my query is answered through E-mail.

The size of the egg seems fine. It should reach 18-20 mm and can then rupture. Was this without any medication ? Do you have irregular periods? Usually the rupture should occur by the 15th or 16th day , but even if you have ruptured around 20th day ( as per your scan ) , it is fine. You can take clomid during the next cycle and do a scan on day 13 . If the follicle is still smallcertain injections like fostimon or merional or puregon can be given.

I just come back from seeing my gyne/obstet. and I had a pap smear, but I am also pregnant. When she done the pap smear I had bleed a little from the vagina area. My gyne mentioned that this should have not happened. I am a little curious, and I wanted to know does this type of thing happen sometimes. note, that I have not been bleeding at all until this. it was not a lot. Is this type of thing normal. I also had a test done to see if my baby would be diagnosed for down syndrome. Th e test come back positive ( I am 34 years old). Does this mean that it is true. but on the other hand, I also had an ultrasound would it had shown on the ultrasound test if there were something wrong or not? My Dr. suggest to me that I have an ultrasound level II done just to make sure. I also read that 98% of the time the test is nor accurate. Inform me on what happens.

Answer

Bleeding while doing a Pap smear is common, so you do not need to worry about it. Please let know the exact result of the down's test. Did you do the triple test ? Were you adviced to do an amniocentesis to confirm or exclude the diagnosis? This would be the most reliable method, rather than level 2 ultrasound.

I don't know what is going on with my body. I have been off of Birth Control for about one year now. Just last month my period was 7 days late from the regular. It has been pretty regular until last month. And also the period itself wasn't looking right either. The color was brown and I may had a half a day of light red. It lastest 7 days with the light brown color. Now this month I was 9 days late. There was a little brown like the way last months first started and it's been day 2 and now there is nothing. No sign of brown or anything at that matter. I took three EPTs but all negative. What is going on? Can you please explain? Besides the fact that I could be pregnant and not know it, what else could it be?

How were your cycles before taking the pills? You are probably ovulating late or not ovulating during some cycles and that is why the delayed periods. However, have your thyroid hormones checked. Also have an ultrasound done to rule out an ovarian cyst. If these are normal, you do not need to worry.

Sometimes, yes. Also, sometimes a follicle is also very large and if this was seen around mid- cycle, it could have been a follicle and not a cyst.

We have been married for 4 yrs. Recently we started to think of having baby. My wife has irregular periods and we consulted the doctor and after seeing the ultra sound report she suggest to have FAS 3 Kit for both of us and then from 2nd day of period have letroz. I read in internet that letroz is an anticancer medicine and should not be used for ovation purpose and that use of this is banned. What should I do?

There have been studies to show that letroz is not safe, so I use clomid . Has she tried clomid? She can start with one tablet a day from the third day of the period for 5 days. You can increase the dose of the clomid to 150 mgms a day . If the follicle is still 14 mm on day 15 of the cycle, take injection pergonal or merional . Repeat the scan two days later and let me know the result. Have you done Prolactin, FSH & Lh hormones on day 2 of the cycle ? If yes, let me know the results.

Usually yeast infection does not cause infertility. However, use polygynax vaginal pessaries to clear the infection. if this does not help, have a vginal smear done to determine the type of infection you have and take appropriate treatment. Relax! The bleeding you had was because you stopped the pill. Have a vaginal smear and a vaginal culture and sensitivity test done to rule out any infection. Let me know the result, so I can advice you further. If the spotting is mid- cycle , it is due to ovulation, so then you can get pregnant.

You can take duphaston three times a day (if your are taking twice a day ) . This will stop the spotting you have. You can take navidoxin tablets if you are vomitting. Do you have a fibroid in the uterus? This can also cause spotting or there can be a hormonal imbalance . However, this should be fine with duphaston. if you continue to bleed, let me know, so I can advice you further. All the best !
