I'm 45 and am on Warfarin for a blood clot and PE. All scans came
back clear except I have a large uterus with a thick lining (in the
family). I also have fibroids and a benign cyst on the thyroid. Before
Warfarin periods were heavy lasting about 9 days. Since a curette and
going on Warfarin bleeding and spotting between periods and very heavy
periods that go beyond 10 days. I had 4 days of heavy floods that occured
mid morning and eased off in the afternoon leaving me exhausted. Gyno had
me on 2 Primolut n a day and allowed me to increase to 3 a day this has
cut period back to a light medium flow but am on my 17th day of period.
Could I take Cyclocaprin or does it clash with Warfarin? My Gyno wants to
insert a Mirena Levonestrel IUS but common side effects are heavy bleeding
and I'm on Warfarin (Coumadin)...I would like to stay alive!!!!!!!!

Many drugs change the efficacy of warfain, but cyclokapron does not.
You can use cyclokapron if necessary, but only for 3-4 days. However,
consult your hematologist prior to this. I don't think Mirena will be very
helpful as it causes a lot of intermenstrual spotting and sometimes takes
a long time to get amenorrhoea (stoppage of periods ). Check with your
gynec whether you should take Zoladex or decapeptyl injections which would
temporarily reduce the size of the fibroids. Since you are now 45 you may
go into menopause soon and you need to worry about your periods only for a
few more years.

You can use canesten vaginal pessaries for 6 nights to reduce the yeast
infection. When you say "you are feeling sick " what do you mean ? Is it

Periods maybe light after clomid , so don't worry. While you were on
clomid , how many eggs did you produce per cycle ? If only one, your gynec
could increase the dose of the clomid to produce two eggs per cycle, which
would increase the chances of conception. You know, however, that you
should be monitored by ultrasound while on clomid. Also, have a PCT (
post-coital test ) done which will determine whether enough sperms are
staying alive in the cervix or not . If the test is abnormal, IUI
(intra-uterine insemination ) may help.

My husband and I have been trying to concieve for 16 months now.
I had a hormone test and I was a little concerned about my FSH level. My
doctor said it was normal but I looked up some info on the web and found
it looked low. I had the FSH test done midcycle (actually a day before
ovulation) and it was 8.0 but I found on WebMD that it should be between
30 - 50 IU/L durring that phase. Is this correct? Thank you so much.

A low FSH level is nothing to worry about and changes from person to
person and from cycle to cycle. Ideally, an FSH should be done on day 2 or
3 of the cycle. If it is high then, it means that the ovaries are not
functioning well. If it is low then, you might need medication like clomid
to help you ovulate.Do a clear plan test to determine whether you are
ovulating or not. If you are not, you can take clomid tablets, under
ultrasound monitoring.

tubes removed. Three months ago I had a cyst on my right ovary rupture.
During my period, I pass very large dark blood clots. Some have been up to
4 or 5 inches long. Is all of this related or is there something else

Do you have fibroids or adenomyosis of the uterus also ? These may
cause heavy periods. You can take cyclokapron from the second day of your
period - one tablet three times a day for 3 days to reduce your bleeding .
This can be used for around 3-4 cycles. If the problem persists, you will
have to see a gynec.

Let me start with, I have a son alomst for now. In June 2001 I
had an Etopic Preg. and they removed the entire tube. Since this I have

You have not mentioned how old your son is and when you stopped breast
feeding. the milk secretion from your breast could be due to a high prolactin hormone. Check your blood for the prolactin hormone, and if
high, take appropriate treatment like parlodel , dopergin or dostinex. A
high prolactin horomone could also prevent conception and once that is
normal, you could get pregnant. You have also not mentioned whether the
ovulation tests showed that you were ovulating or not. If you were not
ovulating in 2-3 cycles, you may need clomid tablets to help you conceive.
You should check whether you are ovulating after the secretion from your
breast has subsided or when your prolactin is normal.

discharge prior to the period, it maybe due to some vaginal or cervical
infection. See a gynec . who can determine whether you have any infection

You can take duphalac syrup for your constipation. Do not eat apples,
drink plenty of water,eat plenty of vegetables and fruits and you should
be fine. You can also take disflatyl tablets - twice a day for 3-4 days to
relieve any gaseous distention you might have. However, if the problem
persists , do an ultrasound again to see what has happened after the
rupture of the cyst.

For the past four days I noticed a strange brown, bloody like
discharge. It is not accompanied by any pain or itching. I am twenty two

If the discharge was around mid- cycle, it is probably due to
ovulation. If there is no other vaginal discharge and it occurs only
around mid-cycle, you don't need to worry. If ,however, it increases or is
present at any other time of the cycle , you will have to see a gynec. who
can determine whether you have any vaginal or cervical infection and give
you appropriate treatment.

I have some questions, first I am a full 42 C bra size, although
my left breast is somewhat larger than my right breast (maybe a 1/4 of a
cup size), is this normal? Last August (2003) I had a small lump in the
right breast, it went away after 3 months, again is this normal? Also I
have very dark areolas, although I am African-American, I am dark carmel
coloured and they are black like charcoal, also they seem to cover a lot

First of all , you have to do the most difficult thing- lose weight!.
Your stretch marks are probably because of the weight gain. You can use
cocoa- butter cream to reduce the marks. The dark skin patches are
sometimes due to certain hormone disturbance like thyroid or due to an
unusual amount of melanin secretion. You will have to check with a
dermatologist for that. Regarding the difference in breast size, you don't
need to worry as it occurs quite commonly, and nothing except surgery can
correct that. However, you must have your breast examined to rule out that
the lump has disappeared. If not, appropriate treatment for that is
required.

The position of your cervix will not affect your chances of conception.
However, lie down after intercourse for around an hour, so that gives
enough time for the sperms to reach into the uterus. If you are overweight
and have associated conditions like PCO or any thyroid abnormalities, it
maybe preventing you from conceiving. However, since you had an LH surge,
you are ovulating. If you plan intercourse around the surge, you should
conceive. The time of the LH surge varies from person to person and cycle
to cycle. Your friend probably had a surge 5 days before her expected
cycle and therefore conceived. She may have ovulated late during that
cycle.

One can get pregnant with PCO. In fact, it is present in 25 % of women,
and some women conceive without any treatment. Do you wish to conceive now

gone away some with out anything, but it still flares up and can be really
annoying although its not there all the time. You wanted me to let you
know what type of discharge I have, I have alot of discharge for a couple

You probably have fungal infection. You can use canesten vaginal
pessaries once at night for 6 nights. Once your infection in the vagina is
cleared, your itching will reduce. Also, do you have a dry skin ? If yes,
use oilatum soap and oilatum shower gel. A dry skin also predisposes to
itching. You can use travocort again for 5 days , but not longer. Also
wear cotton panties and make sure that no detergent remains on them That
could also cause vulval irritation and itching.

Hi! I am a 16 year old female and I have a huge problem. About a
week ago, I discovered a small bump on the left side of my vagina, it was
small and painless so I thought nothing of it. The next day it had got a
little bigger and was starting to hurt a little but it still wasn't
bothering me all that much but by the end of the day it had become quite
painful and had started giving me some difficulty walking so I decided to
put some alochol on it to try to dry it up, it burned very badly. The next
day, it had got even bigger and had turned into more of a blister than a
bump and it was very painful and even more difficult for me to walk, I
wore jeans that day that irritated it and made it hurt even worse. That
night, I discoverd a small brown spot on the top of it that was very hard,
I thought it was turning into a scab but it never did. The next day, it
had not got any bigger but it was getting more painful every day and I
started taking pain pills and using ice packs to alleviate the pain which
only slightly helped. That night, I shaved around the area and cleaned it
very thoroughly and then I put some drawing salve on it as well as some
anti itch/burn cream that seemed to help quite a bit for the time being.
After it wore off, I was in the worst pain and I couldn't even sleep the
pain was so great and by that time I could barely walk. That morning I was
running a slight fever and I was real dizzy when I first got up, I also
felt a little nauseous. When I checked it, I noticed it had got very soft
and it was extra tender and extra painful, that afternoon as I was
kneeling down to pick up something, it popped and quite a bit of brown
fluid came out, the pain went away instantly. Since then, I have had no
problems besides some itching and some discharge with a little blood. I
have not been to see a doctor because I am so afraid of what I will find
out and what will have to be done, I read that the doctor may need a
sample of the fluid to be sent off and tested so I took a sample of my
own, could the doctor use this? My next appointment is Jan. 26. I have
looked up a lot of information on the Internet and herpes always comes up,
my worst fear. I have had a lot of the symptoms of herpes but what I had
looked nothing like the pictures of herpes I saw and I haven't had any
sexual contact in over 6 months and I was tested about 5 months ago, all
my tests came back negative. Is it possible that this could in fact be
herpes? I've also been told it could be nothing more than an ingrown hair
or cyst, what does this sound like to you and what should I do about it? I

It seems like a small abscess on the skin. It could be because of
infection of the hair follicle. It does not sound like herpes- so don't
worry. You can use soframycein skin cream twice a day for 5 days. If it
recurs, you may need oral antibiotics.

Since you have irregular periods, you may not be ovulating or ovulating
late. Ultrasound will help to see your ovaries- whether there is any probblem in it like an ovarian cyst or PCO and also determine when you are
ovulating. Also, it will help detect any other problem like a fibroid in
the uterus. Ultrasound can be done in two ways- one from the abdomen where
you will need a full urinary bladder and would have to drink a lot of
water prior to the ultrasound. If a vaginal ultrasound is done , you need
an empty bladder and the ultrasound probe is passed in your vagina to see
your uterus and ovaries.

Do let me know your blood reports- specially FSH , LH, (both done on
the 2nd day of the period ) , PROLACTIN & THYROID TESTS. Also, your
ultrasound findings- specially if you have PCO. I will advice you
accordingly . Also, do you ovulate? Has that been monitored by ultrasound
or a clear plan test ?

The semen consists of the sperms and other fluids. After intercourse,
the sperms move into the uterus and the remaining fluids of the semen come
out. This is natural and nothing to worry about. However, you should lie
down for about an hour after intercourse, so that there is enough time for
the sperms to go into the uterus. Since you wish to conceive, have your
husband's semen count done and let me know the report. Also, check whether
you are ovulating or not and when you are ovylating by the clear plan test
, which is available in pharmacies.

Since you have been given clomid, you should do an ultrasound around
day 14 or 15 of the cycle to see the size of the follicle("egg") . If your
egg has already come out of the ovary, you do not need the injection. If
,however, it has reached around 20 mm, you may require an injection. The
ultrasound will also let you know whether your ovaries are enlarged
excessively because of the clomid and whether you have any ovarian cyst.
It is not advisable to take an injection without ultrasound monitoring.

normal. If the problem persists, do an ultrasound to rule out any other

blue method. If there is still a faint line, do a blood test- beta- hcg to
check the level of the pregnancy hormone. Sometimes, one gets a faint line
just prior to a period. If the blood test is positive , have an ultrasound
done to check whether the pregnancy is within the uterus or whether it is
an ectopic pregnancy(in the tube )

The cyst may reduce in size or "go away " without rupturing. This
usually happens in a " functional cyst" , which is due to temporary
hormonal disturbances. You would not experience any change in your vaginal
discharge . If, however, the cyst ruptures, you would get pain in the
abdomen.

You can take duphaston tablets- one tablet twice a day for 5 days to
get a period . It is safer than primolut, specially since you wish to
conceive. You must see a gynec who can determine whether you are ovulating
or not . An ultrasound will help determine this and will also rule out any
other reason like PCO or an ovarian cyst which maybe causing the irregular
periods. If yopu are not ovulating, appropriate treatment like clomid
tablets can be given.

Do continue this packet of the gynerAnswer Also, are you taking the pill at
the same time everyday? If not ,and if you have forgotten the pill, you
may get the spotting. Have an ultrasound and a gynec . check -up done to
rule out any other reason for the spotting like a fibroid or a vaginal or
cervical infection or a polyp. If all this is normal, you can change the
pill and the problem might be solved.

If it a D&C, the lining of the uterus is scraped and not frozen.If,
however, she aldo had a cervical erosion, the cervix may have been frozen
by cryosurgery. Since the procedure was done 6 months ago and the vaginal
infection is now, it is unlikely to be the cause. If she has a white curdy
discharge also, it could be fungal infection. She can use canesten vaginal
pessaries, one at night for 6 nights. If the problem persists, let her
gynec see her to determine the type of infection she has and to take
appropriate treatment.

Unfortunately, there is no treatment to get rid of fibroids. If you do
not have any symptoms like excessive bleeding or pain during the periods,
you do not need anything. If you get any of these symptoms or even
pressure symptoms on the bladder, you could use zoladex or decapeptyl
injections- once every 4 weeks to reduce the size of the fibroids. You cn
use 3-6 injections , depending on the reduction in size of the fibroids
after 3 injections. However, the fibroids may increase in size within 3-4
months of stopping the treatment. You can avoid a hysterectomy with these
injections. Once you go into menopuse, your fibroids will shrink.

endometriotic cyst? If yes, you should take decapeptyl or zoladex
injections for three cycles to reduce the size of the cyst and then try

pregnant. I suggest you stop the pill now and once you get a period, star
a new packet. Remember that the bleeding after you stop the pill may not
be your actual period, so for the next cycle use some alternative

tablets- one tablet three times a day for 3 days to stop the bleeding. Do
let me know with which tablets you have had breakthrough bleeding, so I

Do let me know how many eggs you produce( per cycle) with the clomid .
Also, have you done the PCT ( post-coital test )? If this is abnormal, you
would require IUI.You should try IUI with superovulation ( producing 3-4
eggs ) for 3-6 cycles and I think you should be pregnant. If you have PCO
(Polycystic ovaries ) , careful monitoring must be done to prevent
excessive enlargement of the ovaries. If you are not pregnant after 6
months of IUI, try IVF. Relax! Emotional stress also hampers the chances
of conceiving, so the more relaxed you are the faster you will get

Repeat the ultrasound around three weeks after the first one to know
the size of the cyst. Also, check whether the report says whether the cyst
has any projections or "echoes" . This would indicate the type of cyst you
have. It could be a simple serous cyst or an endometriotic cyst. If the
cyst has reduced in size, it was probably a "functional cyst " due to
hormone fluctuations. Do ask your gynec to let you know regarding the type
of cyst and the follow-up required.

Your blood sugar is really high and maybe difficult to control with
diet alone. However, repeat the sugar after 2 weeks of a strict diet
control. If this is still high , you should start insulin injections. If
your blood suar was high when you conceived, there are chances of
congenital anomalies in the baby usually of the spine and the heart. If
the sugar was normal when you conceived, the cahnces of abnormalities is
low. However, the baby can get very large- even upto 4 kgs. by the time
you deliver. This causes problems with a normal delivery as there maybe a
difficulty when the shoulders have to come out (shoulder dystocia) .You
might require a Cesarian section. The baby may have a low blood sugar
level after birth and also changes in calcium and magnesium levels and
thus may require close monitoring by a paediatrician. you might have
repeated urinary, vaginal or skin infections if your blood sugar remains
high. Insulin is absolutely safe in pregnancy , so it would be best to
take it if required.

spotting after intercourse. An erosion of the cervix may cause spotting

require local treatment by an antibiotic vaginal cream. The birth control
pill may stop the bleeding, but sometimes when used over a long period may
increase the size of the fibroid . You can take exacyl tablets- one tablet
three times a day for 3-5 days to stop the intermenstrual bleeding. If
your fibroid increases , you can use decapeptyl or zoladex injections to
reduce the size of the fibroids . This , however, is a temporary effect.
Certain fibroids remain the same size for several years and do not give
problems and do not require surgery. I am not aware of any natural hormone
creams.

around 3-4 months after the last depo. Did you get a period after the last
birth control pills ? If you are planning to conceive and you are not

glucophage and levoxyl. Ultrasound monitoring after clomid is essential to
see when you are ovulating and to make sure you do not form cysts with the
clomid, as this is very common with PCO ( polycystic ovaries).

associated with iregular periods .If you do, check your FSH & LH hormones

First of all, have you been able to sort out the problems you had
earlier which made you have an abortion ? If they are completely solved,
then only should you plan a pregnancy. If they are likely to come up afain,
wait for some time before you get pregnant. Check whether you are

only around 1-2 kilos. Due to the tablets, there is an increase in apetite
also, so make sure you are not increasing your intake of food. You can try
primolut-n twice a day to delay your period as it may cause less water

thickend lining. How thick was your lining and on what day of the cycle
was ultrasound done? Usually, the lining is thin just after a period and

there is endometrial hyperplasia which can usually be corrected by hormone

First of all relax! Genital herpes and warts are very common and are
not "horrible" diseases any more. Treatment is available for both. So,
with adequate treatment like zovirax your herpes can be cured. The small
bumps could be just small abscesses(pus collection) or could be warts. The
abscesses may go with antibiotics. The warts would require cryo-surgery
which can be done in a consultant's office without anaesthesia . Before
you do any treatment, make sure you are not pregnant. If your urine test
is negative, take duphaston tablets- one tablet twice a day for 5 days to
get a period. Since adequate treatment is available, you should not get
depressed about this condition

I had an ectopic pregnancy in July which was surgically removed on July

You can try for a pregnancy now. However, you should be monitored every
month to see which ovary is forming the eggs, as you should try and
conceive only when ovulation occurs in the ovary on the side of the normal
tube . Irregular periods are known to occur with PCOS. If on scan , you
are found not to ovulate, you can take clomid to help you conceive. If you
haven't got a period yet, repeat the clear blue test and if negative, take
duphaston- one tablet twice a day for 5 days to get a period.

Did you have any other problem like a viral infection or were you on

is more than 5 years since your last Pap, you could repeat it.

Do let me know the name of the pill ypu're taking. Also, are you
forgetting the pill or not taking it at the same time everyday? This could
be the reason for bleeding while on the pill. You may need to change the
pill . Do remember , however that the size of the fibroids may increase
with the pill, so regular ultrasound every 3 months should be done. Since
your urine problem is solved, you don't need anything for that. Regarding
the cramps, whenever you bleed or are about to bledd , you may get cramps.
You can take buscopan tablets- one tablet twice a day for that. Once you
don't bleed in between the periods, your pain should reduce. Do let me

my husband but i still do not conceive. I have seen a gyn and had an HSG
test which came out ok. The tubes and the uterus were outlined. With my
cycle, he tells me i must be ovulating. He checked my cervix and he tells
me everything is ok. I feel like i have reached the end of the road and i
really don't know what to do to conceive. I don't want to go clomid as am
not sure of side effects. I am 30 yrs and my husband is 34. Please doc
help as we want a baby soon. Tell me what more tests to go for. Thank you

Check your ovulation by the clear plan test. This will tell you the
best time for conceiving. Has your husband's semen been done? If that is
normal, do a PCT test - Post- coital test. For this test, the gynec. will
initially do an ultrasound and then ask you to have intercourse on a
certain day. You will then be seen around 1 hour after intercourse. The
sperms in your vagina and cervix will be examined to see whether they are
staying alive or not. If the test is O.K. , you don't need to do anything
further, and just try naturally, and you will conceive. If the PCT is
abnormal, you may require IUI ( Intra- uterine insemination ). Do let me
