PCO (Polycystic ovaries)

Question

Hey, i havent had a regular period in over 3 yrz last year i got them once from 27th oct to 4th nov then nuthn i was earlier diagnosed with pcos, now this year i have been bleeding for a couple of days say about 2 weeks sometimes light, not really any odor i some times get a little sharp stab in my vagina area that makes me jump any ideas of what it may be the blood iz brown on day bright red on others and maroon like on otherz it also has clear like sticky stuff through it twice when me and my partner had sex i bled quite alot so we had to stop didnt really know if it waz me or him we are sure it waz me what u think i may have please help? Im worried

Answer: PCO is associated with irregular periods. Since you have had post coital bleeding, have a pelvic examination done to rule out any infection, erosion or polyp in your cervix which maybe giving the bleeding. If there is a problem you would need adequate treatment. Also have an ultrasound done to ensure that there is no cyst ( besides the PCO ) or fibroid in the uterus .Let me know the results, so I can advice you further.

Question

I am 17 and i have p.c.o.s and i was wanting to kno if a home pregnancy test could so up negative n u could really be pregnant but have to have a blood test to have it show up positive

Answer: The pregnancy test would not have any relation to your PCO. It would be dependent on your time of ovulation. It would be positive 15 days after ovulation. A blood test- beta hcg - would be positive 12 days after ovulation. So, if you think or know that you have ovulated after day 16 of the period, a blood test would be preferable. Even if your urine test is negative, you can repeat it a week later and it maybe positive.

B) INFERTILITY

Question

Hello doctor, iam married for last 3yrs,presently iam in australia,my gp here did my ultrasound in jan 2009 which says all is normal(she gave me diane).when i went to india in april 2009 i went to gynec, she checked me personally and said there is no problem,(my gp here said i have minor PCOD)The gynec doctor in india prescribed me with diabex and clomid to be taken from 3rd day i took contraceptive pills till june2009 and from july2009 ihave taken clomid upto nov2009 and daily iam taking diabex,my periods are regular (every month delay by 1,2 or 3days)last i got my periods on dec 10 (before that on nov 7) and in jan 2010 i didnot get my periods, i didnot check for pregnancy test, doc i want to conceive iam 28yrs, shall i wait until feb till my periods show off ,iam scared to chek for preg, doc if it turns out negative.....pls doctor advise if iam taking right tablets. Thanking you in advance

Answer: Diabex and clomid are fine. i presume you are taking folic acid tablets also. I hope you have been doing ultrasound at least once in a cycle around 12th day of the period. This would let you know your fertile period. Also, it would reveal whether your ovaries are forming cysts or getting unduly enlarged due to the clomid. I presume by now you must have got a period . If not, please do a pregnancy test. All the best !

Question

I am trying to conceive 41 days menses i am confused about ovulation at times i am dry and at times wet can u help

Answer: The time of ovulation changes from person to person and even in different cycles in the same person. The best way to determine that would be to do an ultrasound on day 12 of the period. During ovulation , there is an increase in cervical mucus and therefore you would be wet. Once ovulation has occurred , there is a thickening of the mucus, so you would be dry. Dryness also depends on many other factors like stress, so you should be relaxed during intercourse.

Question

Hello Doctor, I'm 33 years 6th months old and my husband is 40. We are unable to conceive as he had a low sperm count of 6 million and this has been corrected by varicoceale with which it increased to 19 million and again decreased to less then 10 million but with 50 % mortility rate. We have been married for the last 4 years and within this time I have undergone one IUI (failure)and IVF with ICSI for which i got my beta hcg result today itself. (0.1 mIU/ml) Non pregrant. It was emotionally devastating. I had 2 ET done on 10.01.2010, one of grade A and the other of poor quality. My doctor said I had 25% chances and I tried to remain as positive as possible. Now my only concern is my increasing age. I will be 34 this november. Will I be able to conceive naturally with my egg quality decreasing every passing day. Do you suggest we consider 2nd cycle of ivf? Please advice as I'm totally depressed and confused. Thank you doctor

Answer: Sorry to hear about your IVF failure. Considering your husband's semen quality and your age , IVF / ICSI would be the best option. In the meantime, you should contact your andrologist and your husband can use some medication to improve his count. If you are unable to do another cycle of IVF at present , IUI should help even with this count and motility. All the best !

Question

I have suffered from irregular periods since age 13, however a doctor adivecd me to induce with mycrogynon, when i stopped i never got periods for 6months, the i was given duphaston for 3months , but after that period i have not seen the periods again for three months now, i want t to get pregnant , what can i do?

Answer: Have an ultrasound done to determine whether you have PCO ( polycystic ovaries ) . Also have your thyroid hormone(TSH ) checked. An abnormality in this can also cause irregular periods. On day 3 of your period, do a serum FSH and LH .. This would also determine what sort of treatment you would require to get pregnant. Let me know the results, so I can advice you further.

C) PREGNANCY

Question

Hello, i have had two miscarriages. After the second miscarriage i got some blood tests done. The results for antibodies showed that my anti-cardiolipin IGM was 3.8 MPL/ml and it should not exceed 3. My doctor has advised me to take Ascard (75 mg) as soon as i conceive next time along with foilc acid. I want to know is it safe to take ascard during pregnancy as it is aspirin and aspirin should not be taken during this time.

Answer: Ascard is a low dose aspirin and has found to give good results in preventing a miscarriage. Several studies have been done and it has been found to be safe in pregnancy when taken as one tablet a day of the 75 mgm tablet.

Question

Dr.Hi firstly we used to consult you when residing in Oman.We have a son named Dhruv ( 6yrs). In between i had 3 obortions and I am carrying for the second baby and am 19 weeks and few days now, i was advised to get the triple marker test done. In the test result we have got the report as trisome-21 at high risk ( ratio 1=239). I have been advised by the doctor to get the amniocentesis done in India and are meeting Dr. Leela lulla on coming tuesday 26/1/2010. Also in the last weeks scan report, all is normal except there is kidney dilation ( both kidneys, mild) which is on border. We would like to have your opinion since we got Dhruv under your observation, checkups and guidance. Awaiting earliest feedback. best regards,

Answer: Your question goes to the omanaccess website and then comes to me without your name. The questions are then sent to me and then I send them the answers after a few days. By now you must have got the result of your amniocentesis. Since you were my patient , you could have phoned me at the clinic directly with your card number. Anyway, you can phone now and let me know the result, so I can advice you accordingly. In case you do not have the clinic number it is 24563181.Also, you can come and see me once you are back, so I can treat you further to try and prevent another miscarriage or preterm delivery. All the best !

Question

I'm 28 years old and I was expecting my period on the 18th of january. the first day of my last period was on 19th of december. I took an at home pregnancy test wich came out positive. I don't want to have the baby and I don't want to have an abortion either. I asked my gyno what I can do to induce a miscarriage and she told me to take Primolut tablets, 2 in the morning and 2 at night for three days. Will this work? Is this dangerous? What else can I do to avoid an abortion if the Primolut is not helpful? Please help me.

Answer: Primolut N is not an abortifacient, so it would not work, unless the fetus was already abnormal or dead when you took it. Certain other drugs are available in different countries. I do not know which country you are from, so you will have to check with your local doctor regarding that. The best way would be to do a D & C , as it assures complete removal. In future, I would advice you to use the pill as the contraceptive measure rather than go for an abortion, as any abortion can cause some damage to your uterus and tubes .

Question

I was pregnant in 2009 and had a still birth at 40 weeks. During the early stage, I was given progesteron injections. Now I am 5 weeks pregnant and the doctor have place me on Primulot Depot injection. I read on the net that the injection has side effects expecially on female babies. Could it have contributed the death of my first baby as it was female baby? What are the risks involved in taking this injection as I do not want anything to happen to my second baby.

Answer: Sorry to hear about your stillbirth.I do not think the stillbirth was related to your progesterone injections. Did you have high blood pressure or diabetes during that time ? Was the growth of the fetus normal ? Regarding this pregnancy, if you are afraid of the primolut depot, ask your doctor if you can take hcg injections- 5000 units twice a week instead of the primolut. Hcg is the pregnancy hormone , so absolutely safe and is found to be helpful in preventing a miscarriage. Why had you been given the injections in the first pregnancy anyway ? Did you have bleeding or any previous miscarriage ? If you have any more queries, please do not hesitate to contact me ..Please ensure that your blood sugar is normal throughout the pregnancy. All the best !

Question

Can u tell me what is the use of taking metaphage 500mg tablet before getting pregnant and having hypothyroid contolled.

Answer: You have not given me a lot of information. I presume, however, that you have PCO and therefore metaphage has been given. In PCO there is insulin resistance which prevents the ovaries from producing eggs. Metaphage helpsthe egg formation. If your thyroid levels a re abnormal, it may interfere with conception. Also, once you are pregnant, if the thyroid levels are not normal, there can be problems with the fetus.
