PCO (Polycystic
ovaries) |
|
Question
Last month i was undergone for an IUI had 2 eggs
of 2.5,2.1.But i got my peroids after 36 days.I have PCO/checked about
FSH 7.2 LH :13 on day 3,if IUI faiured because of LH value.I was scanned
on the 2nd day no cyst and given Ovaral-21.What is my chance of
conceiving in the next cycle? What is LH value?
|
|
|
Answer: Do not worry. Since you did form good eggs, you will
conceive . In PCO the LH value is usually double the value of the FSH,
but this is not the reason of the failure. The success of IUI is 15-20
%, depending on the number of eggs and the sperm quality . Let me know
the semen report and the condition of your fallopian tubes, so I can
advice you further. Do not get disheartened. I am sure you will
conceive. |

Question
I'm 35, married for 41/2 years now. I have PCOD.
My RH factor, TORCH, hysetro-laproscopy and chromosomal analysis tests
are normal. My first preg(natural conception) ended in a miscarriage at
35 days. My second pregnancy(ovulation induction)also ended in a
miscarriage at 35 days(chemical pregnancy). This was my 3rd
pregnancy(ovulation induction) which ended at 18 weeks due to
oligohydramnios. I have never delivered a child before not have I been
able to carry any of the 3 pregnancies to term. This time (3rd
pregnancy) my water broke with very mild contractions & my doc told me
that I had dilated just 1 finger. I had no bleeding either. I was on
tab. Susten & HCG injections till 18 weeks. I avoided sex & travel from
the time I conceived. All I did was normal household work that made me
feel real tired & totally exhausted. I had mild yellowish discharge,
mild UTI & cough which was treated with mild antibiotics for 5 days & I
was also given a single dose of candida tablet for vaginal use. My doc
is unable to tel

Answer: Your question is incomplete. However, I will answer the
part I have received. Sorry to hear about your miscarriages. Since you
have been able to reach 18 weeks, you should be ok in the next
pregnancy. At 13 weeks, have your cervical length checked. If it is
short, you maybe helped by a cerclage. Also, do a vaginal culture and
sensitivity at 14 weeks to ensure that you do not have any bacterial
infection. This can cause rupture of membranes. Regarding the pregnancy
with oligohydramnios, were the kidneys of the baby visualised? Also have
your anti- cardiolipin antibodies checked as sometimes there are
antibodies produced which can cause recurrent miscarriages.Let me know
all these points, so I can advice you further. I am sure you will be
fine during the next pregnancy. |

Question
hi doctor ,i am 24 yrs trying to concieve since
2yrs .my weight is 77,within 8 mnths of marriage i concived and
delivered a unhealthy baby which died after 7 mnths.i was detected with
pcod since 4 yrs.i have very irreglar periods.so i cannot detect
ovulation.in the last six months i was on metformin 500 thrice daily
,then amycordial capsules, hyponidd tablets.last month dr suggested tab
letroz for 5 days from 2nd day of my periods, then i did follicle scan
.the egg ruptured on 18th day.THAT MEANS I AM OVULATING i had relation
on 17 ,18.19,20 day of my period.sill not pregnent.doing this since 3
months.right now i am on no medications. 1.CAN LAPROSCOPY AND D&C HELP
ME?MY GYN WANT ME TO DO THIS. 2.HOW DO I KNOW WHICH DAY I SHOULD HAVE
SEX EVERY MONTH?.PLZ SUGGEST.
|
|
|
Answer: Sorry to hear about the death of your baby. Could you let
me know the cause and what was "unhealthy" ? regarding your ovulation,
it will be difficult for you to judge that without monitoring on scan .
This is because with PCO there is irregular or absent ovulation. Since
you are ovulating with medication, you should continue that. Laparoscopy
& D& C will only be diagnostic. If you do not wish to undergo any
surgery, you can do an HSG ( hysterosalpingography) to check your tubes.
Let me know the result of that. Also, the result of your husband's
semen, so I can advice you further. |

Question
hello doctor, We are trying to conceive for past 1
year with no success. I have a 5 year old baby(normal delivery). My
gynacologist has given me letrofil(2.5 mg) 2 times daily for 5 days from
day 3 of period, tab b-long F 1 daily for 30 days and Capsule Nacfil(600
mg) 1 daily for 30 days. I get regular periods. My age is 31 and my
husband's age is 35. I have read on sites that letrofil has several side
effects. Can you please advice if this is a right course of action? Are
there any harmful effects of the above drugs..Please reply at the
earliest
|
|
|
Answer: I would suggest you first try clomid tablets- on tablet
once a day from the third day of the period for 5 days. Have an
ultrasouind done on day 12 of the period to determine whether you are
ovulating or not and what is your most fertile period. Clomid is safer
than letrofil. I am not aware of the role of nacfil. I presume you have
checked your husband's semen and it is normal. Let me know the results,
so I can advice you further. All the best ! |

Follicle R. O. L.O. ET 1.2 X 1.5 1.2 X 1.6 8MM 1.4
X 0.9 ET Means Endometrial thickness Right Then, what about Follicle R.O
and L.O in MM. How to calculate. I just want to know about MM Please let
me know. I eager to wait for the answer, Please.
|
|
|
Answer: the endometrium should be 10mm , so 9 is ok . However,
the follicles should be 18- 20 mm . You have not mentioned the day of
the period. Also, if the follicles are the size you have mentioned,
certain injections can be given till they reach 20mm. |

Question
Hi, I am 24 yrs old I got married before six
months we r anxious to conceive,we went to gynecologist she recommended
to do thyroid test and usu pelvis scanning then thyroid test is normal
and in scanning i have minicycling poly cyclic ovaries then she
recommended glycomet tablet daily for 10 days,i dont get date correctly
for a month for that she gave naturogest.is there any serious situation
in causing preganancy
|
|
|
Answer: The medication given to you is fine. Polycystic ovaries
are associated with irregular periods. You need to check whether you are
ovulating or not. This can be done by doing a scan on day 11 of the
cycle and following up every 3-4 days till the follicle is 18-20 mm. If
you are not ovulating, check with your doctor whether you require any
clomid tablets during the next cycle. Have you checked your husband's
semen ? Let me know the results, so I can advice you further. |

Question
Dear Doctor, I was having problems in conception
and i was prescribed the following medicines which I and my husband too
and thus I conceived and no wmy son is 3 years old. I want to have a
second baby , just wanted to knwo if i and my husband could repeat these
medicines for pregnancy. the medicines given were : Siphene, T.
proctinal, Becosules, Folvite, Fas, 3 Kit, Bio E capsules, Fertyl M ,
CoQ Could you please guide me. thanks and regards
|
|
|
Answer: Am glad to know that you conceived earlier. However, you
or your husband may not need medication again.Start on folvite. Have
your husband's semen examination done. Also check by ultrasound on day
10 of the cycle whether you are ovulating or not. If both these factors
are fine you may not need anything else.Even if you find you are not
ovulating, you may need siphene. However, close monitoring on ultrasound
should be done to determine your most fertile period and that your
ovaries are not getting enlarged. |

Question
Hi Doctor, I am 28yrs and my husband is 32 and I
have PCOS and on treatment for past 8 months now.During this period, I
ovulted 4 times but could n't conceive(had multiple eggs or no eggs in
some cycles). My husband test is normal and my complete profile is fine
CBC, thyroid, rubella etc. Last month, I changed my doctor and she put
me on Siphene 100mg for 5 days along with metformin twice and folic
acid. I had two follicle on day 12 of size 21mm and 22mm. Doctor gave me
HUCOG injection for ovulation and I ovulated successfully and we tried
during those days and next day from ovulation, doctor put me on
duphaston 10mg two tablets daily for 14 days(morning and evening).Still
could conceive and got periods on 30th day. My periods were also scanty
and very dark brownish in color. Doctor can you please advise me what
could be the problem in conception. I'll be highly obliged if you
respond to my problem. Regards, Shipra
|
|
|
Answer: Since you are ovulating, the medication given to you is
fine and does need to be changed. if your tubes have not been checked,
check them by doing an HSG (hysterosalpingography) and let me know the
result. Try again for 1-2 months and if you still do not conceive,
discuss with your doctor regarding IUI ( intra- uterine insemination).
The period could have been scanty because of the duphaston, so you do
not have to worry about that. |

C) PREGNANCY |

Question
Hello doctor, I am 23 yrs old, married for the
last 6 months. I have pcos, yet managed to conceive without medication.
I am currently 12 weeks pregnant, my doctor has prescribed naturogest
thrice daily along with a hcg shot every 4 days for the first 12 weeks
and once every week until 16 weeks. I am hypersensitive, so she
prescribed asomex 2.5 mg and Arg 9 satchets twice daily. I had an
appointment with her today and she prescribed Loprin 75mg in addition to
what Im already taking. What does loprin do? Is it safe to take during
pregnancy? Do you think I'm on the right medicinal course for my
condition? Please advise. Ultrasounds are normal, cardiac activity also
present and growth seems to be normal so far.
|
|
|
Answer: Naturogest and hcg injections are fine. I usually give
hcg till 12-13 weeks and then stop. Loprin is fine. It is a low dose
aspirin and will help reduce the blood pressure and may help in fetal
growtth. Aspirin in that dose is safe during pregnancy. Was aldomet
given to you for your high blood pressure? That is a safer drug than
asomex during pregnancy. You can discuss that with your doctor. |

marrow transplant. 18 years ago, and pof. in 2005 i got my periods back
for a few months.I have felt ill ever since.ie fibromyalgia. last
september i got my period back. i had a scan on it showed polps. i had
them removed. and they said thats why i was bleeding. not true. i got
another period dec 24. and had my hormones checked. estrogen was 124.
prog 2.3 lh 56. fsh 73. the next month this was the last day of my cycle
which was 39 days. estrogen 173. prog 4.6 fsh 34.8 lh 27. my last period
was light brown and red 3 days. and ever since my breast tenderness has
gone. my doctor wants to give me ellesse duet. plese would you answer
aqnd give me your opinion. nobody seems to want to know. thank you
|
|
|
Answer: First of all, were your FSH and LH checked on day 2 of
the cycle? Do you have hot flushes ? Since you are still getting your
periods, you are in the peri-menopausal stage. If you are asymptomatic,
you do not need Ellesse duet at present. Take calcium tablets. If later,
you do need HRT , I would prefer that you take Livial which has little
or no adverse effect on the breasttissue . |

periods early?
|
|
|

ayurvedic drug. You will need to check with your doctor regarding that.
You can take primolut- N twice a day from today till 29th March, so that
you get a period by 3rd of April , before your marriage. However, check
with your doctor whether you have any contraindications for primolut.
All the best for your marriage ! |

Question
dear doctor, i m 31 yrs old. during 15-20 days of
my cycle at times mucus and sticky type of material is discharged. i
don't know weather it is normal or what? and also tell me on which days
fas-3 kit is to be taken by me and my husband.
|
|
|
Answer: A sticky discharge around mid- cycle is normal and is due
to the hormonal changes around ovulation. Regarding the fas-3 kit , it
would be best for you both to take it at the same time on the second day
of the period. This would rule out the presence of a pregnancy when you
take the medication. |

Question
hi i have had a period but now im having brown
discharge what does this mean
|
|
|
Answer: You have not mentioned when you have got a period and on
what day of the cycle you have had the brown discharge. If the brown
discharge is in the middle of the cycle and only for 1-2 days, it could
be due to ovulation. if otherwise, have a pelvic examination done to
rule out the presence of an erosion or polyp of the cervix or any other
vaginal infection. Letme know the details and the result of the
examination , so I can advice you further. |

Question
i got my period on friday evning of feb 20 and
ended feb 23 during period was lighter than normal and since done been
spotting on the 26 of feb felt really dizzy light hraded and upset
stomach and still felt the upst stomach till know could i be pregnat
even though i just finshed a light period?
|
|
|
Answer: Do a urine test for pregnancy as you could have bleeding
during pregnancy, which is similar to a light period. If negative, wait
for your next period. If it is similar, have an ultrasound done on day 5
of the period to check for an ovarian cyst. If that is normal, have your
thyroid hormones checked. Were you on the contraceptive pill or any
other medication ? These can also change the amount of blood flow. |

Question
Hi, I am 24yrs old and I had my first baby six
months ago. She was planned.I have been on birth control pills since I
was 18yrs old to regulate my periods and am on the pill now. The only
difference is that i'm on Necon. When all my life I used to use Ortho
Novum. So my period cam about a week early (on the pill)on the 10th it
should have been the 16th. So i was off the pill about a week and half.
I am taking the pill now and its the 26th.I was wondering if it is
possible for me to be pregant if i was active w/ husband
14th-16th.Aren't my symtoms too soon. I feel tired and sleepy. I have a
huge headache and nausea.
|
|
|
Answer: Ortho- novum and Necon are similar, so you should not
have any problems. If you got a period while on the pill, it maybe
because you forgot a tablet or did not take it at the same time
everyday. Since you did not complete the pill, you could be pregnant, so
do a urine test for pregnancy. If negative, have your hemoglobin checked
as you could be tired because of anemia. |

Question
Dear mam, I am 25 yrs old and have one boy baby of
2 yrs and 6 months old. By June 2008 I have had oopherectomy due to
severe endometriosis. My left ovary was removed and some of the lesions
from my right ovary and uterus were removed. Then I was advised to take
lupride depot 3.75 mg for the next 6 months. During that time I got the
periods 2 months once. But the bleeding is lesser than usual. After
completing the course in December I underwent a scan in January and no
more cyst in my right ovary. Till now I didnt get my periods. But I am
having severe hip and lower back pain(particularly in right side) since
the past 10 days. Is the pain side effect of the lupride or is there any
chance of chocolate cyst in my right ovary. Please advise me how to
proceed further..
|
|
|
Answer: Did you take calcium tablets while taking the lupride?
Lupride can causea decrease in bone density, so you should take calcium
tablets now for the hip and back pain. Have a scan done to rule out the
presence of a cystin the right ovary. Endometriosis, unfortunately, is
known to recur. Regarding your periods, these are irregular or can stop
while on lupride. You might get a period within 2-3 months after the
last lupride dose. |

Question
Hi doc,my last period was on 16th jan.Had sex and
took postinor2 on 31st jan.Had a light bleeding 5-8.Had unprotected sex
with withdrawal method 19th and 21st.Had a blood pregnancy test on 22nd
which was negative but binding.Took clomid to see my menses. From 25th
brownish discharge and later light bleeding till date.Help pls.
|
|
|
Answer: First of all, instead of relying on emergency
contraception, which has a high failure rate, you should consult your
doctor and use adequate contraception like the pill or the IUCD.The
pregnancy test would be positive 15 days after the conception. You have
not mentioned the month of the dates - 19th, 21st, 25th . Was this
February ? Let me know , so i can advice you further. |
