dear doctor, I am married (1 and half year), 24 years of old.i have become little obese after marriage(w=60kg,h=152).We have been trying to have a baby for the last 4 months. Meawhile i noticed period irregularity.MIssed period feb2008(normal date was 16 delayed for 10 days).Started bleeding 0n 23rd then it stopd again bled on 26th . So consulted a doctor on 26 th feb, was not pregnant. bleeding prolonged till 2rd march ..This month (april 2008)also i missed my period went to doctor i was not pregnant.so doctor told me to took sonography and prescribed the medicine Deviry for 5 days and in sonography found to be Poly cystic.But my period didnt came after taking Deviry so doctor gave me OVRAL*G i have been taking it for last 2 days . The doctor i am consulting does not give proper answer when v ask something about the Desease and medicine and v have totally lost trust in her Please help me doctor to understand my condition and please tell me what to do next ,will i be able to concieve.Or if i conceive

Answer:PCO is associated with irregular periods, infrequent ovulation and obesity. it is present in 10- 20 % of women. Follow a strict diet and exercise regime to ensure that you do not increase your weight. You can take clomid during your next cycle and monitor your ovulation to ensure that you are ovulating with that dose and that your ovaries are not getting unduly enlarged. What is your husband's semen report and are your tubes ok ? All the best !

Dear doctor, I am married for 6 years and have no children yet. I got pregnant thrice , but unfortunately aborted thrice. I have pcod and I am on thyrox. I took syphine and conceived for the third time. Even I was adviced to take aspirin, Weekly hcg injection. Inspite of taking so much of precaution, my pregnancy could not be maintained. For the three times it gets aborted around 8th week. I took number of test after DNC. Aborted tissue has been given for genetic test and they found out that it had TURNERS SYNDROME. We were asked to take Genetic test to rule out genetic deficiency. Both husband and my Karyotype is NORMAL.Even I was asked to take LUPUS ANTI CO-OAGULANT TEST. Even that report was normal. I am so confused. Every single report I took is normal. But no is able to tell me what causes repeated abortions. One doc adviced me to take heparin injection,when i plan for next pregnancy, everyday till the time of delivery. What should I do. All my husbands report is normal.

Answer:Sorry to hear about your miscarriages. PCO is associated with an increased risk of miscarriages. Since your lupus anti coagulant test is normal, you do not require heparin , though , since other medication has not helped, that might help. One of the pregnancies was abnormal, so nothing can be done to prevent a miscarriage in that. have you checked for toxoplasmosis, thyroid hormones and blood sugar ? Any abnormality in these can also cause a miscarriage. Which city are you in ? Let me know all these, so I can advice you further. Answer: Many doctors give Letroz for ovulation induction.The following is the product information regarding letrozole safety.
Letrozole is teratogenic and fetotoxic and causes fetal malformations. The mutagenic and carcinogenic potential of letrozole is not known.
However, many gynecs. are using this drug, so you would have to discus the pros and cons with your gynec.


B) INFERTILITY

Hello Doctor, I am married since May 2004. From last 4 months we have started planning foa baby. But we are unsuccessful. I consulted a gynaecologist here and she asked for 2 tests 1. laparoscopy and 2. semen analysis. Semen report was normal(60Millions/ml). But I hadnt yet gone for laparoscopy. I consulted another doctor and she asked me to take letoval for 5 days(Day 3 to 7)and said that on day 12 we will do a sonography to check ovulation. I took it yesterday. But today as I studied about it on the net, I got to know about its side effects. Now I am totally confused. What exactly is the process that I should follow. Is laparoscopy the first step. Doctor please help me out. Thanks.

Answer: I think you should take clomid and do a scan to determine whether you are ovulating or not and which is your most fertile period.If you are ovulating and do not conceive within 2-3 months, then your tubes should be checked. This can be done by laparoscopy ( which requires anaesthesia and is more expensive ) or by hysterosalpingography ( which does not require anaesthesia and is cheaper). Hi Doctor, I am married for almost a year and wanted to have a baby. I have been taking Duphaston and Chlomid for about 4 cycles. My period always comes three days after taking Duphaston. This cycle however, after stopping Duphaston, I still do not have my period but pregnancy tests are negative. my last period is feb 8 2008. What must be the cause? Thanks

Answer: If you still have not got a period do a blood test- beta hcg. If this is negative, have a scan done to determine whether you have an ovarian cyst. That can cause irregular periods sometimes. Also, have your tubes checked by hysterosalpingography ( which does not require anaesthesia) and your husband's semen. Let me know the results, so I can advice you further. Are you monitoring your ovulation on ultrasound ? That is essential when you are taking clomid, to determine your most fertile period and that your ovaries are not getting unduly enlarged.

 i'm 30 years of age & after 8 months of trying i'm not able to conceive.i visited my gynac & she gave me ovafor,sn15,azitru& metrogyl ..what is the test that can tell whether a lady can become pregnant or not.

Answer: Ovofar should help you ovulate. Have an ultrasound done on day 12 of the cycle to determine that. If you are not ovulating with that dose, you may need a higher dose in the next cycle.There is no test to determine whether you can become pregnant, but the tests would show that you ovulate. Also, have your tubes checked by hysterosalpingography ( which does not require anaesthesia) and this would show that the tubes are patent, so one can conceive. Also, check your husband's semen. Let me know the results, so I can advcie you further.

Hi I am 26 years old. I have had irregular periods for over 7 years now. I took birth control for about 1 year after the birth of my now 8 year old son, and was pretty regular. i stoped taking birth control and basically stoped having my periods. for a few years i had about 2 a year (very light) and then for the a few more years i would have one about every other month. sometimes very very heave for a very long time (like 2-2/12 weeks at a time)and some times very short and very light. or like this month about two weeks ago i started lightly spotting but not every day maybe every other day or every two days very light. I have finally broke down and went to a dr because me and my husband decided we want another child. figured we would have to have some help because for the last 7 years i have not used any form of birth control and never got pregnent. i had test for hormone proublems said everything is in normal range. did a sonagram and reveled thick uterus walls.have a few more weeks till my next appt. but am.....

Answer: Your question is incomplete. However, have your thyroid hormones and prolactin hormone checked. Any abnormality in these can cause irregular periods. Also, have a scan done on day 12 of the period to check whether you are ovulating or not . What is your husband's semen report. Let me know the results of all these, so I can advice you further. I m trying to conceive since last year. me and my husband live in different cities and come together once in a month for a week. this time my doctor prescribed me Polmolute N and Cerephene for increased ovulation. but after using these tabs my own natural ovulation is disappeaed too. plz help me out in this situation.I need my ovulation period immidiately as my off days are about to finish.

Answer: Check your ovulation by doing a scan on day 12 of the period. If you are not ovulating, you will need to take clomid or serophene and monitor the ovulation and try to be together during that time.You have not mentioned whether your tubes are ok and what is your husband's semen result. Are you still taking primolut , and if yes, from which ay of the cycle and for how many days? Let me know all these, so I can advice you further.

hi,i m 30 years and have one daughter her age is 4 years. after my first baby i took a gap with microjinon pills for 1 year after that i tryed 2 get baby and it was so difficult.be4 1 year i got pregnant after treatment with clomid 50 mg one dose.and dimet 4 short period i got pregnant but it was no grow after 3 days i started 2 bleed till all finish. now almost 1 year trying and my dr check me and said that i have 2 take provera 3 corses this is my last one after 3 period. now my period is late 4 7 days i made a pregnancy test 2 times negative .. what u advise me to do. and if i m not pregnant wich medecine do u thing i have 2 take? thanks and waiting 4 reply

Answer: Sorry to hear about your miscarriage. Do you have PCO ( polycystic ovaries ) ? This would give rise to irregular periods and infrequent ovulation. Have an ultrasound done on day 12 of the period to check whether you are ovulating or not. If not, you would need clomid. Also, discuss with your doctor regarding giving " luteal support " . These are medicines given once you ovulate to help the fetus to grow. Some drugs can also be given once you know that you are pregnant. Have you done a toxoplasmosis IgM test ? This would determine whether you have an infection which can cause the miscarriage that you had. All the best !

  I am on first cycle of clomid(3,4,5,6,7),estradiol(8,9,10,11,12morning) and prochieve 8% (after 17th day)..I used the ovulation kit and it showed LH surge on 16th day.As that day was on sunday, my doctor did ultrasound on 17th day and said I had lot of follicles.A week after she checked my Progesterone level and it is 6.8 .....Even though I tood clomid(50mg), my PL level is 6.8...Can I get pregnant with this level?...I am using prochieve now..Today is 27th day of my cycle.I am just hoping to get good news!!!!

Answer: The size of the follicle is important , rather than the number. The follicle must be at least 18- 20 mm. Also, the LH surge test may not be accurate, specially if you have taken clomid. Serial ultrasound monitoring is required.Increase the dose of clomid to 2 tablets a day and have an ultrasound done on day 12 and 14 . If the follicle has not increased by 4 mm, you may require certain injections like fostimon or puregon to help them grow. Once the follicle has ruptured, you may require an injection to rupture them All the best !

I am taking meprate to get periods for past 8 months. i like to have baby now. so i like to know whether i need to stop this meprate. But after stoping this will i get period. Plz clear me.

Answer: If you wish to conceive, you should do a scan on day 12 of the period to determine whether you are ovulating or not. If you are not, you would need clomid in the next cycle. .Once you ovulate, you should get regular periods ( or conceive ) and you would not require meprate.

Dear Doctor, I have been trying to conceive for sometime now. My reports have been normal after i started prescripition for hypo thyroid.My husband has 24 Million per cc. Morpholoy is normal but had low rapid progression. He has been on medication for 3 months. My period ended on 8th March and by 15th i have been feeling quite dizzy, vommiting regularly and feeling weak. Got Beta FCG tested on 1st April that showed 0.86. Please advice if i am carrying. Doctors have said negative and i waiting to see if i would have my periods in the next 4-5 days.

Answer: Check your husband;s semen. If it is normal, you can try timed intercourse- that is - monitor your ovulation by ultrasound to determine your most fertile period and have intercourse during that time. If his motility is still low, discuss with your doctor regarding IUI ( intra uterine insemination ) ..In this procedure, semen is treated with a special medium and this increases the motility of the sperms. The treated semen is then inserted in the uterus. This would increase the chances of conception.Also , ensure that your tubes are patent. All the best !

Hi doc, im 33 trying to conceive for the past 1 yr. My cycle is between 28-29 days. Was trying naturally for 8 months and in treatment for the last 4 months. My prolactin level initially was high. My husbands semen report is good. My doc prescribed BCrip. She also put me on Siphene 50 mg for 5 days in Dec '08 and did follicular study.I ovulated on day 16.had timed intercourse. No luck that cycle. Next cycle she put me on 100 mg siphene. 3 follicles ruptured. I was 10 days late and tested weak positive, but had a chemical pregnancy at 12 days late in Feb '08. Doc adviced me to skip one cycle. She started on 100 mg again, my prolactin level this time was normal.

Answer: Since you have had a chemical pregnancy, you should conceive again. Continue the clomid and ensure that your prolactin is within normal limits.As long as your ovaries are not getting enlarged, you can continue clomid. If your husband's semen motility is low, you can try IUI ( intra- uterine insemination ) to increase the chances of conception. All the best !

Dear Dr. Please answer my query..Please!! I am 32 years of age.My husband is 34years.Married for 4years and I am trying to conceive for 1 and 1/2 years now without success. We went to an infertility specialist and she did tests and told me that my tubes are open and hormonal levels are ok and my husband's sperm count is also normal. Then I underwent three cycles of HMG, ultrasound and timed intercourse.My follicles did develop and ruptured timely but still I could not conceive. Then I stopped the treatment and decided to try naturally for some more time. My doctor has asked me to take Diamet 500mg(twice daily) and folic acid tabs and continue to try naturally for sometime. Now my questions are: 1. How long should I try like this?Is my age is of great concern and I should try medically? 2. How long can I take diamet tabs?Though I am not diagnosed with PCOS still dr. feels that there might be chance of some PCO since I am slightly overweight. 3. Answer: I would like you to conceive before you are 35 , as there is an increased risk of congenital anomalies and miscarriage in women over 35. The best option would be to take clomid tablets ( if you do not wish to take injections ) and have timed intercourse or IUI. .In this procedure, semen is treated with a special medium and this increases the motility of the sperms. The treated semen is then inserted in the uterus. This would increase the chances of conception.Also , ensure that your tubes are patent. Stress can also affect your chances of conception, so relax and you should conceive soon !

Hi Doctor, Last month I had my period on 5th March'2008. As the monitoring started I took Letoval 5mg from 3rd to 7th day. On the 14th day i was given Pregnyl injection and from 18th day i was advised to take Duphaston 10mg twice daily for 15 days. The dose ended on the last saturday i.e. 5.04.2008. My cycle based on 28 days. Depending on that my due date was 01.04.2008. But till the date i am not having my periods. My question is that am i pregnant? Shall i pass through the urine test? Is this delay caused due to Duphaston? How long should i wait? Thanking you.

Answer: If you have not got a period yet, do a pregnancy test. One would get a period within 5-10 days of stopping the duphastn. All the best !

Dear Doctor, I've been married for 3 yrs, bus still have no baby. We've tried to do IVI (intra vaginal insemination) but still failed. My husband's sperm is only +/- 5 million per ejaculate and there's no problem with me except irregular ovulation. What do you think best for both of us? Should we inseminate again? Answer: What is your husband's serum FSH report ? if that is normal, you can ask your doctor to give him some medication to improve the sperm quality. Since you have irregular ovulation, you should take clomid every month to ensure that you ovulate. Monitoring on ultrasound will determine that and your most optimal time for the insemination. Did you mean IUI ( intra- uterine insemination ) ? You have mentioned intra vaginal insemination. If your fallopian tubes are patent, you can try IUI for 3 to 4 cycles after your husband's semen improves. If that fails, then IVF ( in vitro fertilisation ) may help. All the best !

I have a really big concern.I am 18 years old and I am a college student. I have have 2 abortions one in my 10th grade year and my 11th grade year. My first aboirtion I was 3 months and my second one I was 5 1/2 months pregnant.I have been trying to conceive a baby for about a year now.My partner and I have been dating for 6 years and wondering why we havent been able to have a baby since the abortions.We havent used any form of protection for a whole year straight.And I wanted to know if there is still a possiblity that I can still have kids. We have had moments where I thought I was because I had all the signs and such but all of the pregnancy test come out negative.I have a regular period that comes every 28 or 29 days.Please help me to understand.

Answer: First of all, you must have your fallopian tubes checked to see whether they are blocked or not. This can be done by an HSG ( hysterosalpingography ) or Laparoscopy. Also, check whether you are ovulating or not by doing an ultrasound on day 12 of the period. If you are not, you may need medication to help you ovulate. I presume your partner's semen is normal.

 Hi, I am a 28 year female with no children. I had an etopic pregnancy and had surgery on Dec 11, 07 to remove the egg. My periods are regular and come every 28 days. I had a 5 day period in Jan but for Feb my period lasted only 2 days darkreddish blood but I did have heavy cramps on day one. Can I be pregnant? I have had 2 etopic pregnancy,1 abortion, 1 miscarrage now I am ready to have a baby. Please help

Answer:Endometriosis is present in different forms. If your tubes are patent, you can get pregnant with clomid. You have not mentioned where your endometriosis is - ovary, tubes or pelvic cavity. However, monitor your ovulation on ultrasound to determine your most fertile period and also that the clomid is not forming any cysts within the ovary. All the best !

Answer: Since you have had 2 miscarriages, you need to investigate the cause further.Have your thyroid hormones , blood sugar, toxoplasmosis and cardiolipin antibodies checked.

iam 22 weeks pregnant, iam carrying twins. its clomid pregnancy. will ther be any side effects for the babies in future.. as of now my ultra sound says they are ok. it seems twin babies are prematured babies. can u help to prevent the premeture baby.

Answer:There is an increased risk of premature labour with twins. At 12 to 14 weeks ,check the length of the cervix on ultrasound and again labour to see whether you require a cerclage ( a stitch around the cervix ) .Also, consult your doctor regarding giving you certain uterine relaxant drugs. Answer: This should be answered by a dermatologist , not a gynecologist.

Respected doctor, i am 32 yrs old married woman.around 9 months ago i found my right nipple is discharging milk like(sometimes watery)discharging very small amount(if i press the nipple). i notice it in right nipple also but very occassionally. I cheaked my prolactin level. it was 42. doctor adviced me to take 'cabergoline' for three months. i took it. now prolactin level is normal 0.5. But still if i press nipple i am getting very very small( water like color) amount only from one breast. i am scared as i have heard it is a symptom of cancer. A.i am very stressful person. B.i am not taking any contraceptive or any other medicine. C.My nipples are inverted from my childhood. I live in a very remote place. please advice as early as possibe.

Answer: A high prolactin level would be associated with milk secretion. It is not a sign of cancer. You need to keep checking your hormone level every 3 months and whenever it is high , take cabergolin for 2-3 weeks. Stress can also raise the prolactin level. As long as your prolactin is normal, and your breasts have been examined by a doctor to rule out any masses, you do not have to worry. You can do an ultrasound of the breast or a mammography to confirm that there is no other problem.

hi a few months ago i was having very very bad pain on the right side first day of period ... that as gone . but for 6 months now i have had lower pelvic pain and left sided pain never the 2 together it moves around it seems also aching lower back ...and i also spot in between periods . my periods use to be very heavy and alot of clots now they just seems to last longer like over a week . first day of period i dread its even worse and it's bad enough suffering most days so i dread the next period ..somtimes there is pain after passing water this seems to be if i hold it in for a while so i do try avoiding that . also pelvic pressure feeling and seems now ibs is kicking in but i dont know if thats because antibiotics .. i have seen my doctor 2 times he as taken water sample to rule out why there is pain after water ( infection ) it came back clear . he did a full pelvic exam last week and took many swaps because he suspects tube infection on left side . so wating for some results from that . but i am wondering

Answer:Have an ultrasound done on day 5 or 6 of the period to rule out the presence of an ovarian cyst or a fibroid and let me know the result, so I can advice you further. Also, have your urine checked for infection. If you are constipated, take duphalac syrup.

Hello, I have endometriasys so doctor suggested me to take MEPRATE TABLETS - 40 mg daily till 3 months. Please let me know with taking of this much tablet is there any side effect? And also please lete me know why they have prescribed this tablets? How it will helpfull for endometriasys?

Answer: Meprate is a progestational agent and will help to reduce your endometriosis. It may cause nausea and bloatedness and some increase in weight. Have an ultrasound done after you get a period to check that the endometriosis has reduced.

Hello Doctor, I'm 29yrs old, living in UK. trying for baby nearly 6 yrs.My husband's semen test is normal. I have been told in UK its un explained infertility. I had 3 IUI and one IVF all failure in UK.3 months before I went to SriLanka for holiday, and consult the indian gynaecologist there. the doctor told me I have endeo and done the laproscopy . and I've got some severe cervical erosion been treated 50% on. and now I come back to UK , and I have noticed some changes are 1.I have some severe period pain for first two days which I never had it before laparoscopy 2.spotting started 10days before to my period. its brown and pink. doctor could you please advice me I have contact the gyneo in Sri Lanka explain it to her but she said its normal. Doctor is it really normal? after laparoscopy I've been told to wait for 3-6 months time for the results. now it just a month over what can I do to improve my chances to getting pregnant? please advice me thank you thank you very much for your time.

Answer: You have not mentioned whether you have an endometriotic cyst or endometriosis in another area of the pelvis.if it is an endometriotic cyst,have an ultrasound done to ensure that your endometriosis has not increased. The erosion may also give rise to spotting and you may require cryosurgery for that.Let me know the results of the scan, so I can advice you further.

Answer: It does not matter if you took the primolut for a longer period. One gets a period within 4 to 8 days of stopping the tablets. If you still do not get a period , do a urine test for pregnancy to rule out a pregnancy.

My period was late for 2 weeks and i went to the doctor for consultation. as a result, i was given Norcolut Norethisterone 5 mg for 21 days. i did not complete the course because i was very ill. after 3 days, my period comes. that was 2 weeks from my last period which i think is unusual. please kindly advice on what happen and the doctor insist that i finished the course of medicine otherwise the hormones in my body cannot be balanced. what should i do?

Answer:The bleeding you had was not a period. It was bleeding due to stopping the tablets. If you cannot tolerate the tablets, you can ask your doctor to give you some other medication.

Answer:Yes. Liver enzyme–inducing drugs may reduce the efficacy of combined oral contraceptives (COCs), thereby allowing you to ovulate and conceive. You must inform your doctor when you are taking oral contraceptives, so that appropriate drugs can be given.

Hello Doc, I had got IUI done on 29th & 30th Jan. My doc has gvn 18th Feb date for pregnancy test. My periods date is also near. I hv started hvng low back pain and liquid discharge which is in brown color. I scared to go to the doc to hear that my pregnancy is (-). Answer: You would know whether you are pregnant or not by now. One often gets scanty bleeding for a few days before the period. This is normal.

I had a baby 7 weeks ago. I passed lochia for 5 and half weeks and my period began 3 days later. However it is extremely heavy and I am changing every two hours. I have been advised 'Trapic MF' thrice daily for three days to control the bleeding. I am not breastfeeding as my daughter did not survive. Please advise on the medication and also if it is normal to have such a heavy bleeding.

Answer:Sorry to hear about your problem. I am not familiar with trapic mf, so will be unable to comment on that. However, the first period after a delivery is often heavy. If the problem recurs in the next cycle, have an ultrasound done to rule out the presence of fibroids or an ovarian cyst. Also, if you still have some breast secretion, you may need some medication to suppress your milk like dostinex. Check with your doctor regarding that.

 How i had some inffection and my Gyno gave me Polygnax, i used for one day and now am on my periods should i contnoue to use or i should not wait untill there over?

Answer: You cannot use the pessaries during the period.

 i haven't got a real period yet since november, i tried provera once for five days and bleed a little, the following month i had provera twice a day for tendays and still bleed and even less than before - can't even full a 1/4 of a pad. i've taken duphaston after a month and still no changes. what shall i do?

Answer: You may get scanty periods with any progestational drug ( like provera or duphaston). Do you have delayed periods ? Why are you taking provera? If your problem is only scanty periods, that will not be helped with these. Have an ultrasound done to rule out the presence of an ovarian cyst. Also, check your thyroid hormones as an imbalance of these can also cause scanty periods.

I was recently diagnosed with severe anxiety. My doctor prescribed paxel but I would not take it on account I have heard to many bad things about it. So she told me to try st.Johns wort. so for three days Ive been taking it 2 capsules 150mg each three times a day. The first day I started it I got a bladder infection and now that third day I am having a pinkish color when i wipe after going pee and also a brown clumpy kinda like a clot of blood comes out on the toilet paper. Its small and there doesn't seem to be any in the toilet so what does this mean. I heard that st.johns wort helps bladder infections and ive had them before just never with the other effects.(pink color or brown discharge.)What should I do?

Answer: Neither of the drugs are usually associated with vaginal bleeding. You have not mentioned which day of the cycle you bled. If it was around mid- cycle, it could be due to ovulation. Also, have your urine checked for infection or findings suggestive of kidney stones.
