 i m 26yrs old having pcos. ihave a baby girl age 3 by c section.i was taking homeopathy medicines since 6 months.now i went to gyn she told me for scanning to see the size of cyst because i want to take clomid i reduced my weight 7 kg.it was my third day of periods and i was taking 2 tablets of clomids from the 2nd day.doctor told me that i have no cyst and my hormones r balanced so it is the best time to take a chance to get pragnant.but when i went on the 12th day of my period to check ovulation she told me that u r ovulate but there r some poly cycst too ..in that time i felt too much hunger even after eating .after 14 th day of my cycle i m feeling normal hunger as i m fine my periods were regular i just want to know m i pregnant or what is this.pls give me answer soon bye
 

Answer:You will have PCO or polycystic ovaries always. They never go away, so do not worry about that ! What was the size of the follicle ? Did it reach 18 or 20 mm ? If yes, that is fine. if not, you would need to take additional tablets or injections to get the follicles to reach that size. Once you ovulate, progesterone hormone is produced, so your appetite may increase. This , however, does not indicate that you are pregnant. Do a urine test for pregnancy 16 days after you have ovulated. All the best !

 im 26 yrs old,married since 6months.i have pcod diagnosed in 2007.i always have irregular periods.all my tests are normal including thyroid,prolactin,lh,fsh,insulin and others.i had periods for 1month,then i took deviry 10mg for 2months as per doctor´s advise.and then stopped.after that i had regular periods for 4 months.but after that i got married and then i didnt have next period,i again took deviry for 10 days as doctor said.then i had period twice within 1month,and now im having periods after3months and its for 1month again.now which medicine should i take?we dont want to concieve for 1year.i heard about krimson tab.is it good?please guide me.and hows familon and deviry in my case?i just want to have regular periods.

Answer:Since you do not wish to conceive, you can take a low- dose oral contraceptive like Yasmin or Gracial.This will also help you regularize your periods and not allow the prolonged bleeding. I am not familiar with krimson tablets. Did you mean " femulen " instead of familon ? Femulen is a progesterone only pill, so is not as effective a contraceptive pill as Yasmin or Gracial.

 I have polycystic ovaries for the past one year. I am taking treatment for that and after two months the doctor advised me to take ovulation scan. The scan consists of three stages. When should I take this scan?

Answer:The first scan should be done on day 11 of the cycle. Depending on the size of the follicle further scans would be advised. This maybe 2 or 3 times or even more depending on the follicular growth. All the best !

Iam 31 years old and 9 months married , iwant to conceive but did not, my periods are irregular before the it was also irregular it was 07 to 10 days late, after the marriage, periods are 20 to 25 days late, my ultrasound , FSH,LH,TSH,S PROCLACTIN and serium are normal,from the last three years i have the hair growth and acne problem on my face,my skin specialist suggest me aldactone for 4 mtonths,i took aslo the medicine diane 35 three years back but it was not affected on my face hair growth, then he suggest me laser treatment, is the diane 35 tab, effect the general health, can u advise me to conceive

Answer:You have not mentioned whether you have PCO. PCO is associated with irregular periods. hirsutism ( excessive hair growth ) and is a problem which would remain. Since you wish to conceive, you cannot take diane now.Laser treatment is fine. What is yourr testosterone hormone level ? You should now monitor your ovulation by doing a scan on day 12 of the operiod. If you are not ovulating, you would require clomid tablets during the next cycle. All the best !

 Hello Doctor I have had PCOS since my teens. I always skip a month sometimes even two The only time I had three periods in a row was when I got married.I do not have any of the physical symptoms of PCOS Both my children were conceived with clomid tablets. Now that my family is complete does my PCOs problem have to be treated. I have read that PCOS and insulin tolerance are related.I am worried coz both my parents are insulin dependant. Idid not develop gestational diabetes nor have my sugar levels been higher than normal. Both my gynaecs have looked at PCOS from the fertility point of view.what do I do now

Answer:PCO is associated with a higher incidence of diabetes. You must check your blood sugar at regular intervals . If that is normal, you do not have to worry.

Hello Dr, I think you can help me out. Im 28 yrs old & married for last 4 yrs. I was a working women but now a housewife last 2 months. I have PCO from 2005& was on medication since then.My periods were irregular In Aug2006 luckily i conceived and unfortunately it was an ectopic pregnancy & my right fallopian tube was removed in oct 2006. I was not mentally prepared for a baby for a year and now planning for the last 6 months. In the scanreport i took on sep07 there was no pco seen and everything was perfect.I tried conceiving for 3 months, did not work out, later i had to travel abroad with my husband so my doc asked me to take all the test again including hsg. Everthing was perfect except for the scan report stating PCO appearance again. I indeed asked the radiologist who said it was not a classic pco & nothing to worry.Even my doc said the same.My doc who has been treating & monitoring me for so long has put me out of medication for 6 months.Without medicines i was getting my periods on 45 - 50 day cycle.

Answer:PCO is present in 10- 20 % of women and it can never " go away ". It was probably missed in the scan which mentioned that you did not have PCO.You have not mentioned whether you are ovulating or not without medication. If you are, you can continue timed intercourse after monitoring the ovulation on scan. You would also need to see whether the follicle is forming on the left side as only your left tube is present. If you are not ovulating, I think you should take clomid rather than wait for 6 months, specially if you wish to have 2 children before 35 years. All the best !

Dear mam, I have to do IVF with donor egg this month 25-02-08. I had already started my periods. my doctor told me to start meprate for five days thrice a day while my periods. my qts. is what purpose she has suggested me this tab. & when my periods would start after stopping maprate? is it normal to have periods twice in a month by maprate? Tnanks

Answer:Meprate is given as a progesterone supplement . Usually duphaston or cyclogest is given . So, please check with your doctor regarding this.

i am 34 got operated for endometrosis on 21 dec 20007. iam a divorce having no kids. doctor has told me that i have only 2yrs to get married again and concive. i am given lupride injection for three months.why is this injection given what if i get married after 2yrs. wont i concive afterwards pls help will i have any other abdominal problem

Answer:You can conceive as long as you are ovulating. We usually advice that you conceive before the age of 35 years as after that the chances of abnormalities in the fetus increase . Lupride will help to reduce the endometriosis. Let me know the results of your tubes testing ( either laparoscopy or HSG ) and your husband's semen report, so I can advice you further. Don't worry, I am sure you will conceive !

i have got irregular periods since life time and i m trying to conceive since i have started conceiving i get my periods regular like the gap of 10 days but this time i have missed since 14 days and when i did the test on 7 th day it came negative.But sometimes u know u may come to know that u r pregnant after a long time. i m confussed that shall i keep on try to conceive the baby ?if i m pregnant but still it showed negative would it harm the baby in womb? is there still chances to get pregnant if i keep on trying?

Answer:Do you have PCO ? This is associated with irregular periods and infrequent ovulation. Have a scan done on day 12 of the cycle to determine whether you are ovulating or not. If not, you may need medication like clomid. Regarding your urine test- it would come positive 15 days after conception. If you have ovulated later than day 15 of the cycle, it will be positive later. If you still have not got a period, do a blood test- beta hcg. if negative, take duphaston tablets- one tablet twice a day for 5 days to get a periood and then do the scan as adviced . Are your tubes patent and is your husband's semen report normal ?

I am married for past 2 years. I ahve trying to conceive for 1 year. I did 3 cycles of CC nut no sucess. Then a laparoscopy was done. After ovarian drilling, I never got any AF. I have been prescribed with oral contraceptive tablets to start AF and continue with IUI with letrozole. What do you think should be done further to get pregnant ??

Answer:Sorry to hear that you have not conceived. How many follicles did you form with the clomid ? Letrezole is given by many doctors , though it is not approved as an infertility drug and has a higher incidence of fetal abnormalities. However, if your gynec. is comfortable with that, you can try superovulation with IUI. In superovulation, a slightly higher dose of drugs is given to form 2-3 follicles, to increase the chances of conception. However, there is a slightly increased risk of multiple pregnancies. You have not mentioned whether you had any adhesions on laparoscopy and whether both your tubes are patentand also whether your husband's semen is normal or not. If that is fine, you shoyuld conceive with IUI. All the best !

hi doctor, i am 35yrs old. i have been married for 10yrs. i have a daughter of 5 yrs old. i am planning on a 2nd baby.. my gyny gave me 50 mg first..then now i am on 100mg clomid.. follicular study shows good growth of follicles..then gyny gave a jab for it to rupure.. we have intercourse at hte right time..but i didnt conceive..my husband sperm count is high..but only 18% rapid progessive 35 slow progressive..and 40% immotile sperm. my gyny suggesting IUI. i want to conceive naturally. can i conceive naturally with low sperm motility? is IUI a better option? thks u for your time in replying.

Answer:Since the rapidly progressive sperms are slightly low, IUI wopuld be a better option. However, you can try naturally for 3-4 cycles and if things do not work out, try IUI. All the best !

Hi Doctor I am 33 years old and trying for second child. I am having irregular periods from beginning and around 25 years had one son, now trying for second baby. I was using birth control orthoevra and 2006 dec I stopped and from that time 3 months once I get period and during time we were trying to get baby but was not knowing when I was ovulating as periods were irregular. I went to Doctor now , doctor prescibed by Metroxy progesterone and also HCG test , are there chances of getting pregnant please let me know doctor.

Answer:Once you get a period, have a scan done on day 12 of the cycle to determine whether you are ovulating or not. If not, you may need medication like clomid. Also, let m eknow whether you have PCO , whether your tubes are patent and your husband's semen report, so I can advice you further.

hi doctor, i am 29 years old. i am trying to get pregnent for 1year.still now iam not. i consulted an gyn.i've irreguler period.Dr gave me mcbm-69 and duphaston for 4month. that time my period was normal(28th of each month) but i couldn't concieve.the very next month my period on same day but then next month i missed my period. i thot i concieved. but on 45th day i got my period. then for 2months i got period on same day(on 15th)just last month i consulted again to my doc. he gave me folinine(daily 1tab), bigomet sr500 (1tab)daily twise . letroz(3rd day -7th day of my period) and again duphanton(from 17th day to26th day).but i read on several article that letroz is not good and has side effect. my husband semen test result is very good. in year of 2005 my doc. did some hormone test result is like that: TSH--1.76;prolactin--10.16; free tastosterone--2.58. that time i habe irreguler period, so i went to my doc. my hight 4ft11' and weight 53 kilo.plz doc. i want to be a mom. help me

Answer: I am not familiar with mcbm- 69, so I cannot advice you on that. Folinine and bigomet are fine. I usually prefer to give clomid to help one ovulate as it is safer than letrezole. However, your doctor will have to discuss the pros and cons with you'll and then you and your husband can decide on that. Regarding your testosterone and TSH check whether they are in the normal range of your lab. If yes, you do not need any medication for that. If not, you would need apprpriate medication.
The following is the possible problem due to letrezole ( as per the product leaflets ) :
Letrozole is teratogenic and fetotoxic and causes fetal malformations. The mutagenic and carcinogenic potential of letrozole is not known.

Hi, I got married 4 months before. For the first two months after marriage my wife got periods in 30 days cycle. After that, she did not get periods even after 60 days. We consulted our doctor. Doctor did pelvis scan and found her uterus normal and also found follicle growth. she got periods in 73 days. As we are planning for pregnancy, she asked my wife to take letoval for 5 days from 3 rd day of her periods. She also prescribed her geffer (as her HB is bit low) and ebexid (as she look little bulky although her thyroid count is normal - her weight is 70 kg and height is 5.3 feet). Her prolactin is also normal. The doctor has asked her to take REGESTERONE from 21st day of her periods. We had regular intercourse from 10th to 20th day from her periods this time. So we are expecting pregnancy this time. Will taking this tablet REGESTERONE affect baby if in case she get pregnant. Please advice me if she can take this tablet or not

Answer:She does not need the ebexid if her thyroid levels are normal. However, excercise and dieting should be done to ensure that she loses weight. What was the size of the follicle on day 15 of the cycle ? If it is around 18 or 20, have timed intercourse. Monitor the ovulation and if she does not conceive within 3 months, you can try IUI ( intra- uterine insemination ). I presume your semen is normal and her tubes are patent. Also, regesterone is safe. All the best !

Dear doctor, me and my husband we are 37 yrs, old. Had one interruption in 2001 due to personal reasons. wanted to conceive since 2004 but unsuccessful. All reports have been fine so far and cause of infertility is not known( tubes patent by HSG, hormone reports and spermiogram are said to be fine) I am immunhypothyroidic and take 100 Euthyrox every day. Tried Gonal(3 cycles)and Clomid (3 cycles) with natural timed intercourse and 1 IVF but to no success.During IVF two 5-day old blastocysts were transferred on 5th day, but I started having cramps & PMS on the 6th day of transfer itself and bleeding started on the 11th day of transfer. Recent immunological tests show that I have a very high ANA (ANF): 1:320 (normal range for the lab is written to be 1:40). On docs advise taking 7.5 mg aprednisolon/day. Also, Smooth Muscle Abs showed a marked increase of1:80 (<= 1:20)and teh doc. Doctor, I seek your sincere advise as to what u think is wrong with me, and what should be done under the given scenario.

Answer:Sorry to hear about your problem. You are getting the right treatment. If you find IVF difficult to repeat, you could try IUI with superovulation since your tubes are normal. All the best !

Answer:Please send me the semen report in detail, as besides the number of abnormal cells, I would have to know the type of abnormal cells that are present. Also, the coulnt and motility would determine the further line of treatment. You have not mentioned whether your tubes are patent or not. Let me know all these, so I can advice you further.

 hello doctor i am trying for a baby since 1 1/2 year.last time when we gone to india we showed one gynecologist over there.i had laproscopy on that time. findings are bilateral tubel spell++.bilateral ovaries pco(ie early endometric features).diathermy done.utres normal size,?early endometritis. she suggest to take tab siphene 50mg od for 5 days(day 2-day6)for 3 months.then 100mg od for next 3 months+folic acid 5 mg od&metformin 1/2 bd for 1 month,then 1(500 mg)bd for5 months. i had all this tablet but no luck.now again this month we went to india this time she suggest to take tab letoval 2.5mg (D2-D6)for 6 months along with metformin500 mg bd&folic acd 1 od.this time she did the ultrasound on day 10,they found folice is formed,then they repeated the ulra sound on day13.but on that day they can't find the folice. on the day before ulrasound i felt some abdominal pain,so she said it might have reputurd already.i am very much worried now am i on the right way of treatment.can you tell me in such type of case

Answer:The follicle should reach 18 or 20 mm. Was it that before it ruptured ? If yes, then it is fine. Continue the medication during the next cycle and monitor the follicular growth. You can then have have timed intercourse or try IUI ( intra uterine insemination ). You have not mentioned the result of your husband's semen as the further treatment would also depend on that.

My cycle have always been on 30+-2 days. I went to see my doctor for gender selection and had put me in a Primolut N and clomid for one cycle. My cycle are 39 days on clomid. Now I stop seeing the doctor and just want to get pregnant. The next cycle after the clomid was 32 days. I had a 4 days period and spotted until day 9. This is now is over 45 days and still no period. I did do a blood test and it's negative and no cyst. Should I wait for my period to come naturally or take Primolut N? Which is better if I am trying to concieve?

Answer:You can take duphaston tablets- one tablet twice a day for 5 days. These are safe even if you are pregnant. Once you get a period, monitor your ovulation by doing a scan on day 12 of the period and have intercourse accordingly. if you are not ovulating, you would need further treatment for which you would need to see your gynec. is your husband's semen normal and are your tubes patent ?

mam im 25 yrs old and married since 2 yrs but unable to conceive.i got all checks done.everything norml but last year i had lactation and after checks found my prolactin level to be 61/mg.i took b crpt for a month and left it unchecked.now dr adviced to check again and its 44/mg.she prescribed cabgolin.25mg one every week.will this help conceiving and how long does it take to bring prolactin level to normal.

Answer:The prolactin level would be normal while you take the treatment and then rise again once you stop. You should monitor your ovulation and if you are ovulating , you can conceive.


C) PREGNANCY

Dear doctor! I lost a baby (pregnancy) 5 months ago due to birth defets and Downs syndrome...we were told it was a fluke. There were no translocations and both my husband and myself were checked for chromosonal abnormalities. We already have a very healthy daughter. I am having some abdo problems since and I need to do upper GI (abdo x-rays+barium) to look for hte cause of the discomfort I'm having...We were thinking about trying to get prengant again some time int he future. How long after the x-rays can we start trying? I am tramatized with the exposure since I read about birth defects and stuff... Please help!

Answer:Sorry to hear about your previous pregnancy. You can do the X ray during your period to ensure that you are not pregnant. You can try even during that cycle.Please take folic acid tablets for at least a month before you plan to conceive to reduce the risks of other fetal abnormalities like spina bifida and anencephaly. Dear Madam, My wife ( 22yrs, 9month married life) is facing a problem, she got test report as 'Weakly Positive' for pregnancy 3 times in a same week. Her last period date was 31st December 2007, and we went for doctor on 2nd February 2008. From 31st January '08 to 2nd feb she had taken 5 no.s of MODUS-10 tablet ( as she thought its a unregular period ). After test, we went for USG on 5th Feb but report was normal ( as nothing can be shown as pregnant ), so doctor advised us do a next USG after 15-20 days. Suddenly she got bleeding and pain at lower abdoman from 9th Feb evening, doctor gave her a injection for painkilling, as of now there in no pain but continuous periodical bleeding ( though it is not as much as the regular periodical bleeding she expect). We are scared about the same, today 11th Feb Doctor take her blood sample for ELISA kind of test for detail report, and suspecting about pregnancy outer side of UTEROUS. Please advice me what to do? Regards -

Answer:By now you must have got the result of the beta hcg. If there is still no sac seen within the uterus, repeat the beta hcg. if it is increasing there is a chance of an ectopic pregnancy ( outside uterus ). However,if it is still small, she would not need any surgery , but can be treated with methotraxate injection. Your gynec. would be able to advice you regarding that.


D) OTHERS

Hello, I am 33 and was about 10 weeks along when I had a miscarriage. I have an oval hypoechic subserosal leiomyoma that is on the posterior aspect of the uterine body. It is 3.6X3.1X3.7 cm. The endometrial stripe is 10mm. Both ovaries are normal in size and appreance. Uterus is mildly enlarged 10.4 cm lenght by 6.4 cm AP by 6.0 cm transverse. Do you think this is the cause of the miscarriage? Do you think I should have surgery to remove the fibroid in order for me to have a successful pregnancy? Thank you for your time!

Answer:Sorry to hear about your miscarriage. I do not think you should remove the fibroid , as you may carry a pregnancy to term even with that. The only problem with trying to do a myomectomy ( removal of fibroid) is that if you bleed a lot during the surgery, one is forced to remove the uterus. Was it a missed abortion ? If it was, then the fibroid was not the reason for the miscarriage, anyway.

Dear Dr. I was diagnosed to have ovarian cyst and ovarian cystectomy was done this month.i am asked to use Lupride depot injections 1 x 3 monthly.I am on ATT medications fomy chest TB aswell. Is it safe to have both medications at the same time.Please advice me on this.

Answer:You have not mentioned the names of the medication, so I cannot advice you further. As you are aware, many ATT drugs are available, so I need to know which ones you are taking.

Answer: A urine test will clear your doubts about a pregnancy. Breasts maybe tender before a period if you have ovulated. Regarding your infection, this could be a fungal vaginal discharge. Use gyno- pevaryl vaginal pessaries 150 mgm for 3 nights. if the problem persists, see a doctor. You would need to see someone for your anal problem to rule out the presence of hemorrhoids.

I have been diagonised with pcos and did hormone tests which showed i have high prolactin levels. My doctor prescribed parlodel which i started on day 15. Immediately i started parlodel my breats became tender and it is now day 30 and i have not received my periods. My cycle is always 28 and now i dont know what to do. Please advise!

Answer:Do a urine test for pregnancy . If negative, the period maybe delayed due to the high prolactin. repeat the prolactin to determine whether you require a higher dose of parlodel.

I have suffered from thrush twice now and i inserted a perscribed pessery and it has been 15 hrs and it has not come out in a white powdery discharge yet, how long is it that i have to wait to have sex with my partner again without worrying about infecting him or the pessery comming out?

Answer:Some pessaries ( like canesten ) take longer to dissolve. You do not need to wait for the pessary to come out. You can have intercourse 2-3 days after completing the course.

Is there any adoption center in Oman? I'm in my 32nd week of pregnancy.........and will deliver soon in few weeks. Recently I had a big fight with my husband,,,,,,,,and now he's is India............and didn keep in touch for a long time. I have lot of financial problem too,,,,,,,,,He's giving me a sign of divorce. I don want to hand over the baby to him..........so cud u please find me a muslim parent who is ready to adopt my child. Please reply as soon as possible.

Answer: I am not aware of any adoption centre.

Hello. I am 23 years old and i am obese. ever since i started my period it has always been irregular. but these days its getting (i think) way out of control. my cycle always starts with the brown discharge for about a week, then it develops into the full fledge bleeding...then it stays on for months and months at a time. like just recently, it stayed on for the entire month of January then it paused for a 3 or 4 days, came back on around the beginning of the second week of February, and is still on! Mind you, this is all heavy bleeding! i have no pain, no cramps, or any of those complications, my only dilemma is that it will not go off!!! what can i do, what is wrong with me, why is my cycle like this?

Answer:Sorry to hear about your problems . Have an ultrasound done to determine whether you have PCO ( polycystic ovaries) . Your symptoms are suggestive of that. if you are still bleeding, take cycklocapron or dicynone tablets- one table three times a day for 3 days to stop the bleeding. Losing weight may help to regularise your periods. The scan will also determine whether you have any fibroids or any other ovarian cyst. let me know the result, so I can advice you further.

Hello doctor. I am 25 year old unmarried. I have irregular periods, comes once in 2 months and i cannot say when my next period will come. It also lasts for 7 - 10 days. I consulted a gynacologist and she suggested for Transabdominal scan as I may have polycystic ovary. Is PCO a problem? Can this be cured?

Answer:An ultrasound would indicate whether you have PCO or not. There is no " cure " for PCO in the sense that it would "go away ". However, medication can be given to regularise your period and to reduce the flow. Let me know the result of the scan , so I can advice you further.
