Dear Doctor, I am 27 yrs old and got married two years back and was
diagnosed of PCOS.During this time one session of IUI,clomid etc were
tried.Later my Dr advised me to have laproscopic ovarian puncture.I was
on Tab.Deviry till the morning of surgery.On the second day after
surgery I got my periods lasting for 10 days.Again after a week there is
some brown spotting followed by normal bleeding.Dr has advised me to
take Novelon for 21 days starting from today(2nd day).She said on the
2nd day of next period she would give me Clomid. I would like to know
your suggestion regarding the same.My husbands semen is normal.

Answer: The novelon will take care of the irregular bleeding you have
had. Have a scan done on day 12 of the period after taking clomid to
check when you are ovulating. It will also determine that your ovaries
are not getting unduly enlarged. Also have your tubes checked. |

Answer: PCO ( Polycystic ovaries) which is associated with
irregular periods, excessive hair growth and infrequent ovulation. Since
you wish to conceive , have an ultrasound done on day 12 of the period
to check whether you are ovulating or not. If not, you may need clomid
tablets. Again, have an ultrasound done on day 12 to determine which
will be your fertile period. Also, have your tubes checked to ensure
that they have not got blocked after the MTP. You have not mentioned
what you are taking for your thyroid problem. |

cycle , have a scan done on day 14 of the cycle. If the follicle is 20
mm, you would require an injection to rupture the follicle. Check with

Answer: First of all RELAX ! Stress would affect your cycle. Beta hcg
blood test is the most sensitive for pregnancy detection and would be
positive first around 12 days after conception. Urine test for pregnancy
will be positive 15- 16 days after conception and an ultrasound will
only show a sac at 5 weeks of pregnancy ( i.e 3 weeks after conception ) |

hey there, i was just wondering if you could tell me exactly what a
doctor will do to you to find out if you have polycystic ovaries? ive
got all the symtoms, excessive hair on face and body, obese and i find
it very hard to lose weight, irregular periods but the problem with me

been trying to talk myself into it for so long now and its just not
going to happen! secretely ive been waiting for the 'home' smear test to

also get shooting pains in my stomach (low down so i assume thats where

up lying on the floor and i near enough live on nuerofen plus! if you
could give me the step by step guide as to what they will do i would
really appreciate it! can they find out if you have it by blood tests? i
dont want them to make me uncover cause im too overweight and hairy

Answer: First of all, you will need to be treated and an accurate
diagnosis will be required to be made. If you are shy, ask your doctor
to do an abdominal ultrasound to determine the condition of your uterus
and tubes.If you have fibroids in the uterus or polycystic ovaries, that
would be determined by that. However, if you have a " thick " abdomen ,
a vaginal ultrasound will be preferred. You can have blood tests done on
day 2 of the cysle- FSH, LH and Testosterone. Start a strict diet and
excercise plan to reduce weight. It is difficult and a " never- ending
task" , but it will help. Let me know the results, so I can advcie you
further. |

Answer: Have an ultrasound done to determine whether you are
ovulating or not. If you are not, you would require clomid tablets.
Duphaston cannot help you ovulate. It would only help you regularise

with irregular periods. If you are planning to conceive now, have an
ultrasound done on day 12 of the period and check whether you are
ovulating or not. If not, you may need some medication to help you
conceive. If you do not wish for a pregnancy now, you can take tablets
to regularise your periods like diane or duphaston for 5 days of the
cycle to induce a period whenever it is delayed. The advantage of taking
duphaston is that you only need to take the tablets for 5 days instead
of 21 days like diane, which can increase your weight with long term
use. |

Answer: Polycystic ovaries are associated with irregular periods. Have your
serum FSH & LH tested on day 2 of the period. Whenever your prolactin is
high, you would need medication for that. If your FSH is normal, you
would be able to conceive. Anyway, have an ultrasound done on day 12 of
the period and check whether you are ovulating or not. If you are, you
would be able to conceive in the future. Dr Geetha Arjun is a good

repeat your prolactin and if it is high or borderline , you would need

take susten 100mg for 10 days. From 20th day i had verginal infection Dr

Again she satrted my course with Latoval 2.5for five days and susten
100mg from 10th day to 20th day. As every thing was normal and i am
under consultation, i could not understand what was making me away from
pregnency. please suggest me something.

Answer: There are many other reasons why one does not conceive even
though one is ovulating.
Check whether your husband's semen is normal and whethwer your tubes are
normal.A problem in any of these can prevent conception. If both these
are normal, and you do not conceive in 3 months, try IUI ( intra uterine
insemination )

15th day to 10 days). Progynova 2 mg daily 2 (from 5th day to 20th day).
Please give me your valuable suggestion is it proper way to follow this

Answer: Sorry to hear that you have not been able to conceive. You
can try. If your tubes are patent and there are no pelvic adhesions, you
can try IUI again. If not try IVF . Your problem is probably that the
fetus does not get implanted. What is your endometrial thickness at the

glyciphage has been given. Weight reduction helps women with PCO to
ovulate , which they otherwise may not be doing every month. It does not
affect the size of the cyst. Are you taking any medication for
hypothyroidism ? Have a scan done on day 12 of the period to determine

Hi Dr. I have been delayed for 2 months, I was diagnosed with PCOS, then
my doctor gave me Provera for 10 days to take and metformin for 1 month.
I just want to ask if it's not advisable and good if me and my partner
have contact while i am taking those medicines? What will be the
implications if we do sexual intercourse? Also, We are already planning
to have our 1st baby and is it possible within the next 2 - 3 months
that after taking provera and continous metformin i can be pregnant?

Answer: I prefer to give duphaston instead of provera , when one is
planning to conceive , as it is a safer drug. Metformin is fine when you
are planning to conceive. Anyway, once you get a period, have a scan
done on day 12 of the period to check whether you are ovulating or not.
If not, you may need clomid tablets to induce ovulation. All the best ! |

I was on Clomid 50mg.The ultra sound showed nearly 5 eggs. I was
given a HCG shot on December 15th 2007 and IUI was done on December
17th. I took a pregnancy test after 14 days after IUI, and it turned out
negative. And i also took a blood test, and it was negative. Till today
January 16th almost 30 days after IUI, my cycle is not started. We are
confused of what is happening. Any one should happen.Either positive

periods of 28-29 days cycle. I had my periods on Dec 3rd and my doc gave
me Siphene 50 mg 1 tablet for 5 days starting 5th day of cycle, im also

day. But I got my periods on Jan 1st '08. My doc has asked me to take
Siphene 50 mg 2 tablets this time and do the follicular study again. How
long should I do this? My periods has been very regular and hence dont
understand why ovulation is being induced. Also when we make love, the
semen flows out and i donno whether that is common. Please help me.

Answer: If you are ovulating, you do not need to increase the dose of clomid. The increased dose may have been given to try and increase the
number of eggs, to increase the chances of conception. The semen
consists of sperms and other fluids. After intercourse, the sperms go
into the uterus and the rest of the fluids come out- so that is normal.
Have you checked your husband's semen and your fallopian tubes ? |

my MC become 30 for 3 months then 32 for 2 month then 34 to present, I

blood test, Hcg, U/S all seems normal, I read lots of article regarding
womens problems. Then I heard and read on net regarding Clomid, without
any Ob/gynae advised I took clomid CD3-7 for 2 cycles now..and planning
to stop next cycle as I too kinda scared of the side effect....is there
anything I can do to conceive normally without taking any meds? pls.

Answer: You have not mentioned whether your scan revealed that you
have PCO. In any case, never take clomid without ultrasound monitoring ,
as sometimes the ovaries can get enlarged and cause problems. Ultrasound
would also help you determine your most fertile period. have a scan done
on day 3 of the period and if there is no enlargement of the ovaries,
you can take clomid. then have a scan done on day 12 of the cycle to
determine the size of the follicle. Also, what is your husband's semen
report? Let me know these details, so I can advice you further. |

reduce the endometriosis slightly. You should try IVF during the next
cycle. Endometriosis is a recurrent condition, so it would not help much

not getting unduly enlarged due to the clomid.Progesterone hormone is
formed after ovulation and duphaston can both give breast tenderness, so
that is nothing to worry about. Also, the breast pain or cramps would
occur even if you are pregnant. Do a urine test on day 31 of the period

Answer: Was a vaginal ultrasound done? Sometimes, polycystic ovaries
may be difficult to determine on an abdominal ultrasound. Also, have
your thyroid and prolactin hormones checked. You must have a vaginal
ultrasound done on day 12 of the cycle. This will reveal whether you
have fibroids and polycystic ovaries. It will also show the size of the
follicle and thus, determine your most fertile period. If you are not
ovulating, you would require other tablets like clomid in the next
cycle. You can start folic acid, but not metformin without knowing
whether you have PCO( polycystic ovaries ) or not. |

of organs, particularly genital abnormalities. It can also lead to
virilisation of female baby (unwanted hair etc.).it is difficult to
predict which baby can be affected. Since the risk is not unacceptably
high, you both can discuss with your doctor regarding the risks and
decide whether you are willing to take the risk.If you do continue the
pregnancy, do periodic, frequent ultrasound and other tests to make sure
that the fetus is normal. |

in our clinic. Check with the pharmacist. Also, if you still have not
got a period and the test was negative , do a blood test - beta hcg.

before conceiving she was dependent on metformin 500mg. (dose: one
tablet each before breakfast and dinner) till completion of three months
she was continuing the same medicine on doctors advise and till the time
her blood sugar level was fasting-104 and pp-146. now on doctors advise
she has started inj. mistard insuline with 10 units before breakfast and
05 units before dinner. with this her blood sugar level has increased
and are fasting-178 and pp-235. i am worried to see the present level of
blood sugar and request your good advise to complete her pregnancy
period without interruption and also to become father of a healthy baby.
(a) other substitute with which sugar level can be controlled? (b)
whether metformin can be continued during the full pregnancy period? (c)
if inj. mistard insuline is ok to keep the blood sugar level within the
normal limit then the required level of dose to be continued to bring
sugar level. thankyou ~

Answer: Insulin is the best treatment for diabetes during pregnancy. The rise in
sugar levels is because the sugar levels increase as pregnancy advances.
You need to consult a good endocrinologist who can monitor her blood
sugar levels and decide when the dose should be increased or the type
and frequency of insulin changed. Insulin has to be continued till
delivery. Metformin is not advisable now. |

I've been diagnosed of a cervical polyp about 6mm big. I'm 38 yrs old &
got married just about 2 months back. I am already taking some
homeopathic treatment for the past 3 months. I just wished to know if
I'm going have some complications in conceiving. Should I go for the
intercourse or try to avoid it? I tried it about 3 times; although 1st
time it was quite painful during penetration, the next 2 times it was
quite OK. Moreover, I don't have any in-between the periods spotting
problem usually observed in case of similar patients, and my cycles are
also regular. I just want to avoid surgery until I have at least one
child. Please advise.

Answer: The polyp is quite a large size. If it does not reduce with
homeopathic treatment, you must remove it surgically. It is a minor
surgery and you should not have any problems. Removing it would when you
are pregnant, as otherwise you might have bleeding during pregnancy. |
