Question
Dear Madam i am 27 ,married for six months. i am over weight my periods
are normal and regular 28/5.i am trying to concive. plz tell how to find
out weather i have polycsytic ovary? can obesity be the only cause of
PCO seconly while having intercource the semen spills out of the vigina?
is it normal looking forward to advice me on these issues. thanx
|
|
|
Answer: Sorry to hear about your problems. Have a scan done to
rule out whether you have polycystic ovaries. PCO is associated with
obesity and there is no " cause " of polycystic ovaries. It occurs in
10-20 % of women. The semen would spill out of the vagina after
intercourse, it is normal.

Question
i have got married 2 years before, not concieved.i checked my tsh was
high and got corrected, later i had a endometric cyst that has been
removed.but i am trying ,so doctor adviced me to take siphene 100mg.when
i should have relationship after taking tablet
|
|
|
Answer: Did you take medication for your high TSH ? Even if it is
normal now, keep checking it every 3 months to ensure that it remains
within normal limits. Siphene should help you ovulate. Have an
ultrasound done on day 12 of the cycle to see the size of the follicle .
This would determine your most fertile period, which would usually be
between the 12 th and 20th day of the cycle. the ultrasound will also
determine that you are not forming any new cysts. All the best !

Question
Hi mam,I am diagnosed to have P COD,I went to
doctor, she prescribed Glycomet and Reshape tablets,I am not taking
these tablets but i am going for morning walk for 1 hr,what kind of
exercises i should do to reduce my weight. Its bothering me lot
mam.Should i take those tablet,if not, what will happen if i only
continue walking, will it be enough.suggest me to relieve from this
problem.
|
|
|
Answer: I am not aware of the contents of reshape tablets.
Walking is a good way to lose weight together with a low calorie diet.
You have not mentioned your weight and height, so I do not know how much
weight you need to lose. If , however, you have not lost weight in 15
days, you need to see a dietician for that. Regarding glycomet- that is
given to reduce the insulin resistance associated with PCO - not to lose
weight. Also, have an ultrasound done on day 12 of the cycle to ensure
that you are ovulating. If not, you would require some medication like
clomid to ovulate, as diet and exercise alone may not be sufficient,
though they are both helpful. |

Question
Dear Docter, I am 28 yrs old.. My last periods
were on 25th may and i came 2 know about my 2 mnths of pregnancy on 10th
aug07. i had ultrasound evrything was normal than on oct10th i had a
missed abortion (15 wks baby stops growing after 11 wks)and the very nxt
day d&C.. i had ultrasound on 15th oct to make sure everything is
normal..but this report shows a small intramural fibroid involving
posterior wall in lower part, measuring 34*30 mms. By checking all the
reports my Dr. came 2 a conclusion that missed abortion happened because
of imbalance of hormone. My serum progesterone during 1 & a half mnth of
pregnancy was 18.28 so she prescribed me progesterone susten(300 mg)
twice a day till 14 wks of pregnancy. she said nxt time she ll double
the dose. Now I was advised by my doctor to take Torchnil tablets for
three months, two tablets a day and also cabgolin 0.25 mg twice a week
(Dr. tld me that there is a secration from my left breast) My question
is what is the need to take these medications?
|
|
|
Answer: Sorry to hear about your missed abortion. the cabergolin
has been given to reduce the milk production so that your breasts do not
get engorged . I would also like you to check your GTT and thyroid
hormones and toxoplasmosis Igm to rule out other causes of the
miscarriage. I do not have any experience with Torchnil, so I will be
unable to advice you regarding that. All the best for the next
pregnancy. |

hospital past one year for infertility problem. You told us to do
insemination to increase the chance of pregnancy. But my husband not
interested. So,we just did the treatment. Then we went to india for
vaccation, After come back my husband is not allowing me to take
treatment. He is believing the horoscope ane telling after two years we
will get child. But i am already 33 .I am really worrying about my
situation.I want to take treatment only with you.I want to continue my
treatment from January onwards.please you have to help me.I feel really
bad this past 6 months we wasted our time. My husband never understand
the situation I dont know what to do? Pl help me
|
|
|
Answer: Your questions go to the omanaccess website and are then
sent to me without your name for privacy . I do not know what your
problems are . If , however, your tubes are patent and your husband's
semen is normal, you may get pregnant without treatment. Treatment, of
course, would increase your chances of conception. If any of your friends
or relatives can convince your husband to continue treatment , it would
help. If not, you cannot force him to continue treatment. In the
meantime, I will pray that you conceive . Will do my best to help you
once you can come for treatment. |

Question
Hi I just turned 41 I had my tubes burn or clip 6
years ago after having my daughter. My periods have always been regular.
I bleed the whole 7 days since I started my period when I was 12 years
old. My lover and I had unprotected sex. Because of my tubes being burn
or clip. I had my period on the 16 of December it only lasted for 2
days. I have been nauseated and not feeling just like my self. Is there
any way that I can be preagnate. I haven't been since I had my daughter
6 years ago. Please help.
|
|
|
Answer: If your tubes were burnt, it is unlikely that you could
be pregnant, as that causes damage to a large area of the tubes. If,
however, a clip was placed, the clip could have slipped and you could be
pregnant .The failure rate for the Hulka- Clemens clip is around 3.65 %
compared to coagulation which is between 0.75 to 2.48 %.. Do a pregnancy
test. If negative, take duphaston tablets- one tablet twice a day for 5
days to induce a period. If you wish to confirm the condition of the
tubes, you can do a HSG ( hysterosalpingography ) after your period. |

Question
I am 25 years of age, in laproscopy , found that
my left fallopian tube was blocked ( complicated) and right overy
contain adhesion which block the movement of egg to the fallopian tube.
Is there any possibilty for pregnancy ? whether any treatment for
adhesion can be cured or block can be removed
|
|
|
Answer: If the condition of the right tube is fine, you can get
pregnant. However, have a scan done on the 11th day of every cycle to
check from which ovary you are ovulating and only try for a pregnancy
when the egg is on the right side. You can try IUI for 3-4 cycles. If
you do not conceive, you would require IVF. |

Question
Doctor, I am 26 years old my irrguler periods
started when I was 21 years old when I had sexuals intercours but a
comdom was used in the process and every sciene then I have been having
trobule with my mentural. I went to a womens clinc and they gave me a
ultrousand and it came out good. I went to all of my annully exams and
they came out fine to. So they put on brith control pills I have went
through about 7 or 8 differnt kinds they have cause me to gain weight,
excess hair growth, and know they want me to go on a low cab diet I just
do not understand what is going with me. Also, I want to have a hormone
test done they told me that I could not have one because I was on birth
control. But my mine reason for that its becasue I wanted to make sure I
can have a child in the near future. Will this effect my chances of have
a child a couple of years from now?
|
|
|
Answer: Using a condom does not change the menstrual cycle ! I
think you must be having PCO ( polycystic ovaries ). This is seen in 10
to 20 % of women and is associated with increase in weight, acne ,
hirsutism ( excessive hair growth) and irregular periods. This can be
detected on ultrasound. Once you stop the pills , wait for your normal
period in the next cycle. Then do a FSH and LH hormone test on day 2 or
3 of that cycle. Let me know the results, so I can advice you further. |

Question
Dear Doctor, Iam 28 yrs old. I have got my
abortion done on Nov 3rd 2007. I havent got my cycles yet. The urine
pregnancy test is faintly positive even after my abortion. I have got my
ultrasound scan done. But no gestation sac is seen. The scan report says
the endometrium thickness is 16 mm and endometrial hyperplasia. Is this
a serious problem, will i be able to conceive or an I conceived again.
|
|
|
Answer: The urine test would be faintly positive after the
abortion , as it takes time for the hcg hormone to be completely
eliminated from the body. Repeat the pregnancy test and if negative, you
are not pregnant . You can then take duphaston tablets- one tablet twice
a day for 5 days to get a period. If you do not wish to conceive now,
please take an oral contraceptive pill like Yasmin from the first day of
your period. Ensure that you do not have any contra- indications to the
pill before staring it. Before starting the pill have an ultrasound done
to ensure that there is no hyperplasia. |

Question
I have a history of irregular periods and have been using the nuvaring
for over a year to regulate my cycle. (I have my tubes tied so don't
need it for birth control). For the past few months I have had spotting
and irregular bleeding (about 4-5 weeks straight at one point). I have a
history of irregular paps then normal paps (they alternate about every 6
months so no former testing has ever been done). I have lower abdominal
pain but I also have interstitial cystitis so that really isn't new at
all. Please let me know what diseases, conditions my symptoms might be
indicating. I am seeing my GYN but would like to go there with some
knowledge. Thank you! |
|
|
Answer: If you do
not need contraception, you should stop using the nuvaring.
The spotting & irregular bleeding could be due to that.
Have you done an ultrasound recently? Have that done to
ensure that you do not have a fibroid or an ovarian cyst.
Also, a pelvic examination would reveal that you do not have
a cervical erosion or polyp which can also cause the
spotting. Have you checked your thyroid hormones? An
abnormality in these can also cause irregular periods. Also,
if your periods are not very irregular and you get them
without medication, why do you want to take medication to
regularise your periods ? Irregular periods are not harmful
to the body as long as they are not associated with
endometrial hyperplasia and other problems. If you still have
doubts after your Gyn examination, let me know the results
of all these tests, so I can advice you further.

Question
Hai doctor, I am 34 years old, married and with
two kids. The age of younger one is 7 years. Since, six months i am
suffering from severe abdominal pain during mensuration period. I
consulted the Gynocologist, she told that some fibroids are there at the
wall of uterus. she suggested to take Leupride injections for 3 months.
My question is, will i have any side effects with the injection and will
it be curable with this treatment. Thank you very much in advance. |
|
|
Answer: Fibroids can cause a lot pf pain during the periods.
Lupride will temporarily reduce the size of the fibroids and since there
will be no periods till you take the 3 doses of the injection, you will
be relieved of the pain also. However, the fibroids would increase in
size after 2-3 months of stopping the treatment. You have not mentioned
the size of the fibroids. You can let me know the initial size of the
fibroids and their size after 3 months of treatment and I can then
advice you further. Regarding side effects- you can get hot flushes,
night sweats, vaginal dryness. irregular spotting, tenderness/pain at
the injection site .. It can also cause headaches, weakness, increased
urination, breast tenderness ,nausea/vomiting, changes in weight, hair
loss, and acne.Extended use of the drug (more than 6 months at a time)
is not recommended due to the risk of bone density loss/osteoporosis. |

Question
I'm 48 yrs old i haven't had a period since 5/05.
Since the first of Dec 07 I've had this discharge that is light brown
and light yellow along with redness but no smell or itching. It flows is
constantly using both tampons and pads. any suggestions? I thought I had
a vaginal infection and used otc 3 treatment. Which didn't change
anything. Thanks
|
|
|
Answer: You need to have a vaginal swab examination done for
culture and sensitivity to determine the type of infection you have and
the appropriate treatment. Also, have an ultrasound done to ensure that
you do not have a fibroid or an ovarian cyst. A pelvic examination will
also reveal whether you have a cervical erosion or a polyp. Once your
infection is settled, have a Pap smear done if you have not done one
recently. |

for the eggs not to form well. All the best ! |

Question
Mam, We have been trying to have a baby for the
past one year. Recently we got a chek up done. The following was
inferred. I have a slightly high Lrolactin level little more than
normal. 2) I have tiny follicles in both my ovaries, which have been
diagnosed as suggestions of PCOD. I am having regular periods and am not
over weight. I have been given Bromocriptine, which I have been having
for the past 15 days. I am still not preg. PLease let me know whats the
next thing I need to do. |
|
|
Answer: Bromocryptine will help reduce the prolactin level and
help you conceive. However, have a scan done on day 11 of the period to
ensure that you are ovulating. If not, you would need clomid tablets to
help you ovulate , which you can continue with the bromocryptine. Also,
are your tubes patent and is your husband's semen normal ? |

Question
Hi, I got my period on Dec 8th and I think I
ovulated on day 10 or 11. Then on day 20 of my ycle I started spotting
light brown.. Not much at all, Not even enough for a panty liner. Still
spotting on day 21 and now on day 22 still spotting brown not a lot
though.. The only thig different is I noticed the discharge had a few
things in it like a little snot hear and there.. I am confused !!! I was
wondering if I might be Implantation? We are trying for a baby.. I took
a few HPT but all Negative.... Help me please?? Thanks
|
|
|
Answer: If you still have not got a proper period , do a blood
test- beta hcg. if this is below 5 , you are not pregnant. Have a pelvic
examination done to determine whether you have a cervical erosion or a
polyp, which maybe giving you the spotting. During the next cycle, have
an ultrasound done on day 11 of the cycle to determine when you are
ovulating. All the best ! |
