PCO (Polycystic
ovaries) |
|
Question
Hello Doctor am 27 yrs married for 2 yrs with
pcod.my doc put me on metformin and folic acid.i have done all the other
tests everything is normal,my husband's semen is also normal.my dr gave
me letroz last month when i went for follicular study my size 2.4mm on
the 12th day and increased to 3.5mm on the 15th day? what should i do?
should i go for a laparoscopy? or continue taking letroz? pls advice |
|
|
Answer: You can continue taking letroz and monitor on ultrasound
to determine your fertile period. Have you checked your tubes? If not,
you can do a hysterosalpingography ( X ray ) to determine whether your
tubes are open or not. Laparoscopy would determine the same, but that
involves surgery. If the tubes are ok, you can try IUI ( intra uterine
insemination ) for 2-3 cycles. All the best ! |

Question
Hello doctor, Last year in June 2009 my PCOS was
determined in US pelvis scan. After that doctor prescribed me for taking
T-Janya(pregnency contorl pills) for 6 months along with some vitamins
and iron tabs.So during 6 months my cycle was regular and i was feeling
very fit and fine. But when i stopped medicine i got regular periods for
3 months with very pain to my vaginal and abodemen side. Within this 3
months,i usually faced indigestion, vomiting etc before 10-15 days of
period date. So once again i gone through some all the hormonal tests
and US pelvis scan. US pelvis scan ---detected the PCOS in both the
ovaries. LH-12.0 FSH-5.13 PRL-13.45 T.Testo-32.8 FBS-73 PPBS-86
F.INSULIN-4.52 PP.INSULIN-10.08 T3-77.8 T4-8.5 TSH-1.62 CORTISOL(SAM)-12.67
DHEA-S-->122 Doctor has prescribed me for taking Krimson 35,with that
Melmet SR(500) and Aldactone(25).. Please let me know how these
medicines will be effective,is there any side effects. Please let me
know whether PCOS will be there |
|
|
Answer: PCOS is a condition which does not go away.It is
genetically determined. However, the medication that you have been
given would help in regulating your periods. You have not mentioned
whether you wish to conceive now. If so, you can stop the krimson
35.When one ovulates, one does get indigestion and vomiting before
periods. While you were on the pill you did not ovulate, therefore,
maybe you did not have these symptoms. I usually do not give pills
for a very long period. You can get regular periods by taking
duphaston for 5 days rather than the pill for 21 days. |

Question
Hello mam. I am 25 years old married girl.I came
in UAE 1 year before and started putting weight upto 20 kg more.My
doctor dignosed the PCOD and for the past 6 mnths treated with
metformin. But i could not reduce my weight. This month my doctor gave me
clomid. But she did not advised me for follicular study. Now i heard some
side effects of clomid.can i proceed with this tablet? |
|
|
Answer: PCO is associated with increase in body weight and
irregular periods and infrequent ovulation. For the weight
reduction, you would have to do dieting and exercise. Clomid would
help you ovulate and conceive. There are side effects of clomid, but
if your ovulation is monitored well on ultrasound, there should not
be any problem. You must have ultrasound monitoring done anywhere .
All the best ! |

Question
I am 29 years old and i am suffering from
polycystic ovaries. From the beginning of my cycle i had irregular
periods and it was always 40-44 days cycle. In July 2009 my hormone
tests revealed high prolactin, and after taking tablets it has come back
to normal. Last month i took siphene 50 mg, and my follicle size on day
19 was 29mm on day 28 when i took home pregnancy test a pink faint line
appeared appeared but however i got my periods within 2 days.At present
i am on the second cycle of Siphene 100mg along with Premarin.625mg on
day 10 my follicle size in my right ovary was 10x7 whereas there is no
follicle growth on the Left ovary. I am really worried, is it really
hard to conceive with polycystic ovaries? does the present cycle atleast
appear hopeful? is Siphene really working on me? |
|
|
Answer: PCO is associated with irregular periods and
infrequent ovulation. As long as your follicle is reaching 20 mm,
you do not have to worry. If in any cycle the follicle is found to
be small, injections like puregon or fostimon or gonal f can be
added. Also, have your prolactin hormone checked. This may have gone
up again and you may require treatment for that. Let me know the
results, so I can advice you further. All the best ! |

B) INFERTILITY |
|
Question
I was 6 yrs married im 28 yrs old my husband is 35
yrs old i have i child now 1 girl she is 4 yrs old this comming july 28,
since im single my periods is iregular, until now same iregular im
trying to lose my weight but still not getting pregnant again i take
clomid dotex and other medicine provera primolot still not get
pregnant, now i take the orgametril to the blood coming still not come my
meanstration i make pregnantcy test 2 times but negative... always like
this.... im tired to much!!!! |
|
|
Answer: First of all, relax ! Have an ultrasound done on day
12 of the period to determine whether you are ovulating or not. If
you are not, you would require to take clomid in the next cycle or
increase the dose if you do not ovulate with the given dose. Then
repeat the ultrasound in the next cycle. That would determine your
most fertile period. If you are ovulating and not yet conceived, try
IUI ( intra uterine insemination ). Have you checked your fallopian
tubes by hysterosalpingography or laparoscopy ? Let me know the
results, so I can advice you further. |

Question
Hello Doctor, I am 26 years old.married for an
year..no issues yet.i had irregular periods and recently i was also
diagnosed with thyroid. subclinical hypothyroidism.and i have been
taking thyroup 25mcg and i have completed a course of ginette and the
doctor asked me to take divery 10mg from 18th -24th day calculating
first day of my periods as 1..but i again got periods on 15th day
itself. it continued only for 2 days. when i called them, they asked me
to take the same tablets calculating the 15th day as 1..and to have the
tablets from 10th-20th day.. First the tablets were for 7 days..now the
tablets are for 11 days..why? the doctor is a thyroid specialist in
india.we are planning to have a baby. except for in appointment dates i
am not able to speak to the doctor directly..only to people working
there.i am scared. |
|
|
Answer: I think you also need to see an infertility
specialist. I would prefer that you take duphaston rather than
deviry when you are planning to conceive. You have to continue your
thyroid tablets, anyway. All the best ! |

Question
Hi im 23 yrs old n my boyfriend n i have been
trying to concieve for the past yr but the problem occurs cuz i havent
had any periods since we started having sex. i was until we started then
they went away. i went to a doctor and she prescribed me some birht
control pills to build up my hormones.im not going to lie i stop taking
them so where my period started it stopped again now its been 6 months
since my last menstrual cyle. in my past when i was 18 i had a
miscarriage due to stress since then i have been unable to get pregnant.
i also have a rare blood type of o negative. does this all mean im
unable to get pregnant |
|
|
Answer: Have blood tests- serum FSH and LH done on day 3 of
the period. If these are normal, you can conceive. You should then
see an infertility specialist who would give you clomid to help you
ovulate. You would then need to monitor your ovulation on
ultrasound. Also, have your partner's semen examination done. Let me
know the results, so I can advice you further. Your blood group is
not a problem for conceiving. |

Question
Hi Doctor, I am a 29year old in my 7th week
(6w3d)of my 2nd pregnancy. The first was normal and had no problems at
all.I had a vaginal scan yesterday after noticing a slight brown
discharge (only once and only little). The doc advised that the baby's
heartbeat was fine and growth also. However, the gestational sac was not
large enough. She suspects hormonal imbalance and has put me on
75mgAspirin, 20mg Duphaston (10mg morning & night) for 30 days and 4
injections of Pregnyl (over 4 weeks). I am also on regular 5mg folic
acid. Please let me know you thoughts on the meds being taken and if i
should worry! FYI - i mostly have a late cycle ranging from 32-38 days,
which is different every month. Thankyou. |
|
|
Answer: The treatment given is fine. Have a scan done at
12-13 weeks to determine the growth of the baby and to rule out any
abnormalities and also do a nuchal thickness which would determine
whether there is a higher incidence of down's syndrome etc. Hope you
have stopped bleeding by now. |

Question
Dear Doctor, Hi how are you im 33 yrs old dh is 32
we have been ttc for 1 yr now i had a tubal lygation reversal done by dr
michael cooper here in australia fantastic dr anyway i had bloods done a
wed 31st april my hcg was 3 ml my gp said i was pregnant how is that
when its concidered neg under 5 ml anyway i ended up being late for
mensis and when they did arrive i spotted pm sat sunday 1230 pm i
started bleeding heavy by monday afternoon it slowed down and was brown
today tuesday brown spotting when i wipe pad quit clean i have never had
a period so short i did have heavy bleed for 1 and a half days than
brown discharge i normally have a 5 to 7 day cycle would you have any
idea what is happening? i also had 3 faint positive urine tests what do
u think i should do |
|
|
Answer: Usually 5 and above is considered positive for
pregnancy. However, that changes in different labs, so you have to
determine the cut off value in your lab. Sometimes, there is a very
low hormone ( hcg ) produced by the fetus giving weakly positive
urine tests also. These fetuses are then not capable of growing . |

Question
why is there hair growth on the face & body? what
should i do lessen the growth. especially my diet? let me know |
|
|
Answer: Increased hair growth on the face maybe due to a high
testosterone hormone. Have your testosterone hormone checked. If it
is high, you can certain medicines to reduce it. However, the effect
of these medicines is temporary. A high level is associated with PCO
( polycystic ovaries ). Have an ultrasound done and let me know the
results, so I can advice you further. In the meantime, you may
require laser hair removal. There is nothing in the diet which can
modify that. |

Question
Dear Doctor, I'm 22 years, unmarried. I'm having
irregular periods due to hormone imbalance problem. My gyn adviced me to
take Meprate for 3 Months. Starting at 5th day continuing to 21 days (No
of days 21). I'm getting periods within a week of completion of this
tablet. I feel i've put up weight. Can you please advice me some
exercises to reduce weight.. |
|
|
Answer: Meprate can cause an increase in weight. You can
consult your doctor to try and reduce the days of meprate to only
5-7 days( from 15th day ). This would still get you a period without
much weight gain. |

when I'm having intercourse with my husband I urinate. I also urinate
when I pass gas, laugh, cough, or sneeze. Why is this and what can I do

Answer: You need to see your gynecologist, who can examine
you and decide whether you have laxness of your vaginal muscles
which maybe causing these problems. Appropriate exercises then can

Question
Hello Dr.how to take duphaston tabs. Is it 3x a
day or 2x a day? Please advise |
|
|
Answer: This depends on what you have been given duphaston
for. Please check with your doctor. |
|
|
|
Question
I am ovulating and the 4th day of ovulation, I
took duphaston and feel something like menstrual cramp. I know it has
something to do with egg rupture, but what does that mean? Are my eggs
still fertile for conceiving? Or does that mean I will be having my
menstruation soon? |
|
|
Answer: It is difficult to answer this question as I have not
seen you. The menstrual cramp is usually on the day of ovulation. If
you had some more follicles almost of the same size as on the day
you ovulated, you could have had a second follicle ruptured within a
day or two of ovulation. Anything after that would be due to any
cysts which may have formed. |
